    "description": "<h2>Salary</h2>Starting at $19.00 / hr\n<h2>Overview</h2>\n<p><strong>Career Opportunity: Use Your Clinical Experience in Community Health </strong></p>\n<p><strong>Community Engagement Specialist, a this </strong><strong>Clinical-Adjacent Role </strong></p>\n<p> </p>\n<p><strong>Full-time hours required for approximately three months with the opportunity to transition to the Elderwood Network at project completion.</strong></p>\n<p> </p>\n<p><strong>Support Complex Patients in the Community.</strong></p>\n<p> </p>\n<p>If you have with experience supporting medically complex or vulnerable populations and are interested in expanding your impact beyond traditional bedside care, this opportunity offers a unique way to apply your clinical experience in a community health and care coordination environment.</p>\n<p> </p>\n<p>As a Community Engagement Specialist you will work directly with members in their homes and communities, helping identify barriers to care, supporting social determinants of health (SDOH) interventions, and assisting interdisciplinary care teams in improving outcomes for complex populations.</p>\n<p> </p>\n<p>This is a clinical-adjacent role where your healthcare background helps strengthen member engagement, quality initiatives, and operational support.</p>\n<p> </p>\n<p><strong>Special Project Opportunity:</strong></p>\n<ul>\n <li>This role supports a full-time community health initiative with Elderwood IPA for approximately three (3) months.</li>\n <li>Following the project, employees may have the option to transition into per diem LPN opportunities through Elderwood Staffing Solutions, Elderwood’s internal float pool serving facilities throughout Western New York. These roles offer flexible scheduling and the ability to pick up shifts ranging from occasional coverage to part- or full-time hours.</li>\n</ul>\n<h2>Responsibilities</h2>\n<h1>What You’ll Do as a Community Engagement Specialist:</h1>\n<h3>Community & Member Support</h3>\n<ul>\n <li><p>Conduct in-home and community visits to assess member safety, stability, and overall well-being</p></li>\n <li><p>Identify and document changes in member condition, environment, or service effectiveness</p></li>\n <li><p>Help address social determinants of health (SDOH) by connecting members to community resources</p></li>\n <li><p>Promote member education, engagement, and independence in managing their care</p></li>\n <li><p>Communicate observations and concerns to Care Managers and interdisciplinary teams</p></li>\n</ul>\n<h3>Care Coordination & Clinical-Adjacent Support</h3>\n<ul>\n <li><p>Assist with documentation review to support care planning, audits, and quality initiatives</p></li>\n <li><p>Support gaps-in-care identification and follow-up</p></li>\n <li><p>Participate in care coordination workflows and escalation processes when concerns arise</p></li>\n <li><p>Monitor member satisfaction and service delivery concerns</p></li>\n</ul>\n<h3>Quality & Operational Support</h3>\n<ul>\n <li><p>Participate in audit readiness activities and quality improvement initiatives</p></li>\n <li><p>Review dashboards and reports to identify trends and service gaps</p></li>\n <li><p>Assist with data validation and quality follow-through</p></li>\n <li><p>Provide operational support across Care Navigation and Quality teams as needed</p></li>\n</ul>\n<h3>Documentation & Professional Practice</h3>\n<ul>\n <li><p>Maintain accurate documentation in the electronic medical record (EMR)</p></li>\n <li><p>Follow HIPAA, Medicare, and Medicaid compliance requirements</p></li>\n <li><p>Adhere to safety protocols during community visits</p></li>\n <li><p>Participate in team meetings, case reviews, and required training</p></li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>Ideal Background for an Community Engagement Specialist:</strong></p>\n<ul>\n <li><p>Minimum two years of experience working with complex healthcare populations</p></li>\n <li><p>Experience supporting Medicare, Medicaid, MLTC, or similar populations</p></li>\n <li><p>Experience working with frail, elderly, or chronically ill individuals preferred</p></li>\n <li><p>Comfortable conducting home and community visits</p></li>\n <li><p>Reliable transportation required</p></li>\n <li><p>Experience using EMR/EHR systems</p></li>\n <li><p>Bilingual (English/Spanish or other languages) preferred</p></li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
