    "requisitionDescription": "<div><p id=\"isPasted\">Join Hopestone Hospice and make a meaningful impact where it matters most&mdash;providing compassionate end-of-life care to patients and families in the comfort of their homes.</p><p>As a Hospice RN Case Manager, you will lead and coordinate individualized patient care plans while delivering expert clinical support, comfort, education, and advocacy.
