Description

Family Support Counsellor Contract Up to 18 months (35 hours per week) Internal/External Posting The Alzheimer Society of Peterborough, Kawartha Lakes, Northumberland and Haliburton is seeking a compassionate and skilled professional to join our team as a Family Support Counsellor. In this role, you will make a meaningful difference in the lives of individuals living with dementia and their care partners. Through supportive counselling, education, and connection to services, you will help people navigate complex journeys with confidence, dignity, and support. About the Role As a Family Support Counsellor, you will build trusted relationships with individuals and families, providing ongoing counselling, education, and guidance. You will support clients in understanding dementia, developing effective care strategies, and navigating health and community services. You will also facilitate support groups and collaborate closely with community partners, including participating as a member of interdisciplinary teams in memory clinics across the region. Key Responsibilities Provide supportive counselling to individuals living with dementia and their care partners Conduct standardized assessments and develop individualized service and care plans Facilitate psychoeducational support groups Provide education on dementia care practices to promote safety, reduce care partner stress, and prevent crisis situations Support care partners in responding to behavioural expressions of dementia Assist with future care planning, advocacy, and system navigation Coordinate client participation in Alzheimer Society programs and services Build and maintain strong relationships with community partners Collaborate with internal and external interdisciplinary teams Represent the Alzheimer Society on committees and community initiatives Who You Are You are an empathetic and relationship-focused professional who values a person-centred approach. You are confident managing a dynamic caseload and are comfortable balancing multiple priorities. You’ll thrive in this role if you: Enjoy building meaningful, supportive relationships with clients and families Value both independent work and collaboration within interdisciplinary teams Enjoy working in the community, including travelling to homes, clinics, and other community settings Qualifications Diploma or degree in Social Work, Social Services, Geriatric Care, or a related field (or equivalent experience) Preference given to regulated professionals in good standing with an Ontario regulatory body Experience Supporting individuals living with dementia and their care partners Assessment, care planning, and service coordination Working in healthcare, human services, or inter-professional environments Skills & Competencies Strong knowledge of dementia care and person-centred practices Excellent interpersonal and communication skills Strong organizational and time-management abilities Cultural sensitivity and awareness of diversity Knowledge of community, health, and social service systems in the region Proficiency in Microsoft Office and client databases Additional Requirements Valid G Class driver’s license and access to a reliable, insured vehicle Vulnerable Sector Police Check (within the last 2 months) Membership in a relevant regulatory body is an asset Working Conditions This role involves working in a variety of settings including office, clinic, community, and client homes. Travel throughout Peterborough, Kawartha Lakes, Northumberland, and Haliburton is required. Occasional evening and weekend work may be required. Our Compensation Package Grade 4 position with starting salary commensurate to experience: $22.00 - $27.00 per hour (35 hours per week) Access to the Employee Assistance Program ( *excludes LTD ) 15 days annually (prorated for contract roles), accrued monthly, in addition to office closure between December 25 and January 1. 5 paid and 5 additional unpaid emergency leave days per year In the summer months, flex time may be used to take Friday afternoons off making the most of the great weather and everything our four counties have to offer.
