Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire. Job Description The ECM   Case Manager - Care Coordinator is responsible for assessing the strengths and needs of clients enrolled in the Permanent Supportive Housing (PSH) program. This role includes linking clients with appropriate resources, developing individualized housing and service plans, and monitoring progress toward the goals established in those plans. The ECM Case Manager empowers clients on their path to self-sufficiency by providing trauma-informed care, practicing active listening, and supporting client engagement. They play a key role in connecting clients to essential services, including medical and mental health care, substance use treatment, financial assistance, and other supportive programs. In addition, the ECM Case Manager is responsible for documenting client progress and maintaining accurate records in alignment with organizational standards. This position requires the ability to maintain strict confidentiality and handle sensitive information with professionalism, tact, and diplomacy. The pay range for this position is $24.00 per hour The schedule for this position is Monday - Friday, 9:00am to 5:00pm Responsibilities Maintain a caseload of up to 35+ individuals, providing consistent and individualized case management support. Collaborate with Care Team members—including Care Coordinators, primary care physicians, and other healthcare providers—to ensure integrated, client-centered care. Connect ECM members to essential social services and supports, such as transportation, food assistance, and behavioral health resources. Coordinate discharge planning by working closely with hospital staff to ensure smooth transitions of care and continuity of support post-discharge. Develop comprehensive service plans by evaluating household needs, setting measurable goals, and conducting regular reassessments to ensure progress and relevancy. Conduct regular client engagement, including home visits, room checks, house meetings, and wellness checks for clients in both Micro-Communities and scattered-site housing. Support income generation efforts by ensuring clients are actively engaged in job searches, benefits applications, skill-building programs, or other employment-related activities. Advocate for clients by attending appointments when necessary and supporting their access to housing, healthcare, education, and emotional wellbeing resources. Coordinate transportation for client appointments when deemed necessary to remove barriers to access. Encourage participation in life skills programming, therapeutic interventions, and community activities that promote independence and personal growth. Ensure housing habitability standards are met through routine inspections for cleanliness, safety, and compliance with local housing regulations. Identify and connect clients with appropriate community-based services and resources that meet their evolving needs. Ensure medical coverage by connecting each household member to health insurance and a primary care provider. Participate in regular supervision sessions (individual and team) to review client progress, develop service strategies, and enhance program implementation. Collaborate with Housing Specialists and internal departments at Illumination Foundation to ensure clients successfully retain permanent housing. Provide emotional and logistical support to clients during move-ins, including preparation of supplies and conducting supportive check-ins throughout the transition Assist with annual recertification, guiding clients through documentation gathering, form submission, and adherence to program requirements. Maintain accurate, timely documentation of all client interactions and services in the Homeless Management Information System (HMIS) and internal EMR (KIPU), ensuring compliance with reporting and privacy standards. Expectations Use a personal vehicle to conduct home visits to Micro-Communities, scattered sites, and fulfill other organizational transportation needs as required. Respond promptly and professionally to phone calls and emails from clients, colleagues, and external partners to maintain effective communication and service continuity. Ethics & Professional Boundaries Maintain ethical and professional boundaries at all times while delivering client services. Exercise sound judgment and adhere to best practices in all interactions, ensuring client safety and dignity. Uphold strict confidentiality of all work-related information and client records in accordance with HIPAA and organizational policies. Demonstrate the ability to perform under pressure, meet service targets, and maintain a high level of productivity. Show flexibility in adapting to shifting priorities, client needs, or organizational changes. Preferred Experience/Minimum Qualifications Required: Bachelor’s degree in Social Services, Healthcare, or a related field; or a minimum of two years of equivalent training and relevant experience. Basic computer proficiency, including the ability to send and receive emails, manage documentation, and summarize data using spreadsheet tools (e.g., Excel). Valid California driver’s license and a clean driving record. Must be eligible for company vehicle insurance coverage. Possession of a reliable personal vehicle with current and valid automobile insurance. Preferred: Bilingual in Spanish, with the ability to communicate effectively with Spanish-speaking clients. Experience in non-profit housing or housing programs that support individuals with disabilities and chronic health conditions. Minimum of one year of experience working with at-risk and/or homeless individuals in a supportive services or case management capacity. Familiarity with Management Information Systems (HMIS) and data entry for client services. Experience using KIPU, an Electronic Medical Records (EMR) system. Knowledge of and experience with the Housing First model, harm reduction approaches, and de-escalation techniques. Proficiency in Microsoft Office tools, including Outlook, Word, Excel, and Calendar.
