Our staff practices patient-centered care by serving each patient with a personalized care plan that meets their individual needs. &nbsp;Our patients have access to support services that include, nutrition, health education, case management, pharmacy, lab, and x-ray at our health center locations. &nbsp;East Valley serves the health care needs of uninsured and underserved individuals and families throughout our 8 health center locations. &nbsp;</span></p></div><div><p><span style=\"font-size: 16px;\">Our mission is to provide access to excellent health care while engaging and empowering our patients, employees, and partners to improve their well-being and the health of our communities.</span></p><p><span style=\"font-size: 16px;\"><strong><em><u>Position Purpose:</u></em></strong></span></p></div><div><div><div class=\"fr-view\"></div></div></div><div><div><div class=\"fr-view\"><p><span style=\"font-size: 16px;\">The Family Planning Counselor/CPSP Case Manager provides counseling tailored to each family planning patient and educate them on human reproduction, contraceptive methods, pregnancy testing, and pregnancy options. This position will be responsible for assessing the patient&rsquo;s needs and making appropriate referrals. The Family Planning Counselor/CPSP Case Manager provides information to programs and services for prenatal care, health education, and nutrition education.</span></p></div></div></div><div><div><div class=\"fr-view\"><p><span style=\"font-size: 16px;\"><strong>Family Planning Counselor Responsibilities and Functions:</strong><strong><em>&nbsp;</em></strong></span></p></div></div></div><div><div><div class=\"fr-view\"></div></div></div><div><div><div class=\"fr-view\"><ul><li style=\"font-family: arial, sans-serif;\">Screens emotional stability and gives pregnancy test results and provides pregnancy counseling.</li><li style=\"font-family: arial, sans-serif;\">Provides educational instruction on contraceptive methods in accordance to Health Education Protocol for Family Planning.</li><li style=\"font-family: arial, sans-serif;\">Assesses patient&rsquo;s educational needs and tailors counselors session accordingly.</li><li style=\"font-family: arial, sans-serif;\">Reviews tobacco use assessments provided to Family Planning patients.</li><li style=\"font-family: arial, sans-serif;\">Conducts teen risk assessment for adolescent patients.</li><li style=\"font-family: arial, sans-serif;\">Makes necessary referrals to on-site professional counselors and off-site services as needed.</li><li style=\"font-family: arial, sans-serif;\">Performs eligibility screening for all Family Planning patients.</li><li style=\"font-family: arial, sans-serif;\">Completes State eligibility forms on Family Planning patients.</li><li style=\"font-family: arial, sans-serif;\">Collects and reports statistical information.</li><li style=\"font-family: arial, sans-serif;\">Attends workshops, conferences, and/or meetings, which Clinic Supervisor/Manager consider necessary for fulfillment of duties.</li><li style=\"font-family: arial, sans-serif;\">Other duties as assigned.&nbsp;</li></ul></div></div></div><div><div><div class=\"fr-view\"><p><strong>CPSP Case Manager Responsibilities and Functions:</strong><strong>&nbsp;</strong></p></div></div></div><div><div><div class=\"fr-view\"></div></div></div><div><div><div class=\"fr-view\"><ul type=\"disc\"><li style=\"font-family: arial, sans-serif;\">Provide CPSP orientations via one (1) hour face to face session.</li><li style=\"font-family: arial, sans-serif;\">Provide Prenatal Combined Assessment/Reassessment Tool and Combined postpartum assessment per CPSP guidelines &ndash;
