This role focuses on supporting daily structure, routine, and overall well-being through consistent presence, engagement, and reassurance.&nbsp;</p><p><br></p><p><strong>ESSENTIAL JOB FUNCTIONS</strong></p><ul><li>Follow the participant’s person-centered Health and Wellness Plan&nbsp;</li><li>Support the participant in maintaining their daily schedule and routine&nbsp;</li><li>Provide reminders and cueing for activities of daily living as outlined in the Health and Wellness Plan&nbsp;</li><li>Provide medication reminders as outlined in the Health and Wellness Plan&nbsp;</li><li>Encourage use of language assistive devices as appropriate&nbsp;</li><li>Serve and participate in meal planning and encourage adequate hydration&nbsp;</li><li>Promote and participate in physical activity appropriate to the participant’s abilities and preferences&nbsp;</li><li>Support participant safety and awareness when outside the home and in the community.</li><li>Encourage participation in community activities that the participant desires including faith community attendance, indoor and outdoor walks, shopping, and meals&nbsp;</li><li>Arrange rides and accompany the participant utilizing ESLC Transportation, Uber, Lyft, and other community transportation services&nbsp;</li><li>Support the participant in maintaining a tidy home environment&nbsp;</li><li>Complete and submit a detailed note and checklist at the end of each shift&nbsp;</li><li>Consistently uphold the participant’s dignity and treat all individuals with respect</li></ul>",
