As a Primary Care Registered Nurse (RN), you will play a pivotal role in providing comprehensive healthcare services to patients within a primary care setting. You will work closely with physicians and other healthcare professionals to deliver patient-centered care, focusing on prevention, wellness, and disease management. Your expertise will be essential in promoting health education, assessing patient needs, and coordinating healthcare services to ensure optimal patient outcomes.<strong> Shifts:</strong></p>\n<ul>\n <li>Gold Team Shift (1 FTE): 0730-1800 (10-hour shift, closed every Wednesday)</li>\n <li>Purple Team Shift (1 FTE): 0800-1630</li>\n <li>Platinum Team Shift (1 FTE): 0800-1630</li>\n</ul>\n<p><strong>Location: </strong>Atlanta VAMC, 1670 Clairmont Rd., Decatur, GA  30033</p>\n<h2>Primary Responsibilities</h2>\n<ul>\n <li>Patient Assessment and Care: Conduct thorough initial patient assessments, including health history, physical examinations, and symptom evaluation. Monitor and manage patient conditions, addressing acute and chronic illnesses, and collaborating with physicians on treatment plans.</li>\n <li>Health Education and Counseling: Educate patients and families on preventive care, wellness strategies, and treatment options. Provide guidance on medication usage, nutrition, and lifestyle modifications to promote overall health and well-being.</li>\n <li>Care Coordination: Coordinate patient care activities, including referrals to specialists, diagnostic tests, and follow-up appointments. Ensure seamless transitions between healthcare settings and maintain communication with interdisciplinary team members.</li>\n <li>Documentation and Record Keeping: Maintain accurate and detailed patient records, including medical histories, symptoms, treatments, and outcomes. Adhere to regulatory requirements and organizational policies for documentation and reporting.</li>\n <li>Patient Advocacy: Advocate for patients' rights and preferences, ensuring their voices are heard in healthcare decision-making processes. Serve as a liaison between patients, families, and healthcare providers to facilitate open communication and shared decision-making.</li>\n <li>Quality Improvement: Participate in quality improvement initiatives to enhance patient care outcomes and operational efficiencies. Contribute to evidence-based practice guidelines and clinical protocols.</li>\n <li>Emergency Response: Assess and respond to medical emergencies within the scope of practice, providing immediate care and initiating emergency protocols as needed.</li>\n</ul>\n<p> </p>\n<h2>Minimum Qualifications</h2>\n<ul>\n <li>Licensure: Current, unrestricted RN license from any state in the United States.</li>\n <li>Certification: Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certificate accredited by the American Heart Association.</li>\n <li>Experience: Minimum of three (3) years prior experience in a direct care setting (e.g. outpatient setting, medical-surgical setting); preferred to have worked in this setting within the last six (6) months</li>\n <li>Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by ACEN or CCNE.</li>\n <li>Ability to perform under pressure in emergency situations.</li>\n <li>Excellent documentation and communication abilities.</li>\n <li>Team-oriented with a focus on collaborative patient care.</li>\n <li>Ability to complete and successfully pass any required government background check</li>\n</ul>\n<h2>About Aptive</h2>\n<p>Arrow ARC supports Veterans Health Administration facilities and offices across the U.S. with health care staffing and program support via the 10-year Integrated Critical Staffing Program (ICSP).
