The Therapist II will share their knowledge base through:  serving as a clinical instructor for students, mentoring new staff, and participating in community education activities. The Therapist II will participate in performance improvement and program development activities. </p>\n<p> </p>\n<p> </p>\n<p>Bachelor’s degree in Physical Therapy or entry level equivalent is required.  Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state.  Current CPR card.</p>\n<p> </p>\n<p>Schedule</p>\n<p>Full Time, Days</p>\n<p>$30,000 Sign-On Bonus </p>\n<p>Contact me if interested at [email protected] or schedule a call with me to discuss at</p>\n<p>  https://MemorialHealthIL.as.me/KariWatson</p>\n<p> </p>\n<p><strong>To review Memorial's Benefits click here: Benefits - Memorial HR</strong></p>\n<h2>Qualifications</h2>\n<p>icensure/Certification/Registry:</p>\n<ul>\n <li>P.T. license in the State of Illinois required.</li>\n <li>CPR Certification preferred.</li>\n <li>If applicable, must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (5 year MVR will be required).</li>\n</ul>\n<p> </p>\n<p>Experience:</p>\n<ul>\n <li>2 yrs of experience required.  </li>\n</ul>\n<p> </p>\n<p>Other Knowledge/Skills/Abilities:</p>\n<ul>\n <li>Achieves & maintains 11-20 points on the Clinical Ladder Grid.</li>\n <li>Basic computer skills required.</li>\n <li>Ability to clearly and effectively communicate with patients, families, peer and other team members as needed for assessment, intervention, and professional development. </li>\n <li>Must be physically able to lift, support, move patients 50 – 100 lbs.</li>\n <li>Universal Precautions Category I.</li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Perform Patient assessments with patients in a specific service area.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Assign a physical therapy treatment diagnosis.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Establishes plan of care with objective, measurable goals.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Provides interventions supported by evidenced based practice in accordance to the established plan of care.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Assesses the effectiveness of treatment and modifies treatment to achieve goals.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Plans for discharge.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Communicates with patient, family, caregivers and members of team to promote maximum benefit of care.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Provide patient care documentation that meets quality standards and supports ethical billing practice.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Demonstrates ability to organize workload and manage time effectively.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Maintains positive working relationship with patients and referral base.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Demonstrates teamwork by routinely offering to help others or assisting others whenever needed.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Demonstrates ability to handle difficult and/or stressful situations appropriately.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Shares knowledge through service as a clinical instructor and mentoring new staff.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Shares knowledge with department and community through participation with educational offerings.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Performs other related work as required or requested.</li>\n</ul>\n<p> </p>\n<p>The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job.
