    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><table border=\"1\" cellspacing=\"0\" id=\"isPasted\"><tbody><tr><td><p>Position Summary:</p><p>Responsible for the full client care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients&rsquo; health and human services needs.&nbsp;</p></td></tr><tr><td><p>Position Description:</p><ul><li data-pasted=\"true\">Under minimal supervision, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered</li><li>Maintain caseload of medication management clients, necessitating quarterly contact and occasional community visits. Primarily an office-based position.</li><li>Handle case assignments, draft service plans, review case progress and determine case closure for review and/or in consult with supervisor</li><li>Deliver on-going services to clients which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting,housing assistance, and referral and linkage to community resources as needed</li><li>Assist in helping clients achieve wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.)</li><li>Develop and/or implement individualized client treatment or service plans and ensures compliance with applicable standards and agency policies</li><li>Document all clinical activities to meet specific program,funder and credentialing entity criteria timely and with accuracy</li><li>Participate in treatment team and assists in making recommendations for client status changes</li><li>May assist in providing education and/or group curriculum</li><li>Adhere to professional standards as outlined by protocols, rules, and regulations</li><li>Complete agency, accrediting body, and funder required training&nbsp;</li><li>Other duties as assigned</li></ul><table border=\"1\" cellspacing=\"0\" id=\"isPasted\"><tbody><tr><td><p>Required Knowledge, Skills and Abilities:</p><ul><li>Ability to learn case management principles, healthcare management, and reimbursement requirements</li><li>Excellent organizational and time management skills</li><li>Effective verbal and written communication skills</li><li>Ability to relate to and interact with a wide variety of people</li><li>Advanced level problem solving skills and ability to multi-task while prioritizing</li><li>Ability to utilize technology including electronic service documentation system</li><li>Basic computer skills with ability to navigate Microsoft applications including but not limited to Word, Outlook, Excel, Teams. and Power Point&nbsp;</li></ul></td></tr><tr><td><p>Job Requirements:</p><ul><li data-pasted=\"true\">Associate&rsquo;s degree in behavioral science or related field, or equivalent work experienceas allowed/required by regulatory bodies</li><li>&nbsp;Driver License and valid auto insurance</li></ul></td></tr><tr><td><p>Essential Functions/Physical Demands:</p><ul><li>Positional: Driving in accordance with job duties assigned.
