Aging services and post-acute care continue to undergo major transformation, and HDG understands the opportunities and challenges inherent with these uncertain and unsettling times. Our experienced consultants create customized approaches specific to each client’s unique circumstances to ensure they are well-positioned and equipped to navigate the landscape of change in post-acute and senior care This search is for a client of HDG. CareChoice Cooperative in MN. The Director of Clinical Development will work collaboratively as a part of a leadership team to develop and implement key elements of a standardized Model of Care (MoC) in participating skilled nursing facilities. This MoC will be supported by the development of a remote clinical coordination system designed to facilitate timely clinical intervention and support for patients at risk of hospitalization. The primary role of the Director of Clinical Development will be to identify and implement innovative strategies and clinical solutions to support project efforts. This will include developing standardized care paths, transition processes, communication protocols, support systems function and management of the clinical coordination team. An advanced clinical skill set specific to the nursing home environment is essential. This is a 1.0 FTE position with competitive salary and benefits. This position requires a combination of work from an office in Roseville, MN and regular travel to participating locations within the Minneapolis/St. Paul, MN metro area. This position will work collaboratively with a contracted project management team and report to the CareChoice Medical Director Special Knowledge, Skills And Abilities Demonstrated ability to influence/lead change. Excellent conflict management skills. Skilled communicator with ability to form strong interpersonal connections with a variety of colleagues. Ability to work independently with minimal oversight and recognize when and how work should be performed. Ability to analyze data to discern relevant trends and identify resulting action needed. Understanding of post-acute care reimbursement methodologies. Working knowledge of post/acute care EMRs, particularly Epic, Matrix, Point Click Care Working knowledge of State and Federal regulations in the nursing home environment. Practical working knowledge of computers and related office software (MS Office, SharePoint) and equipment We are a values driven organization and offer a competitive wage and benefit package.
