The Care Concierge is on-site at Senior Living Facilities and is a bridge between patients, providers, and Curana’s virtual support team  </p>\n<h2>Essential Duties & Responsibilities</h2>\n<p>Patient Support</p>\n<ul>\n <li>Conduct monthly in-person rounds for patients enrolled in Advanced Primary Care Management (APCM) or the Guiding an Improved Dementia Experience (GUIDE) programs</li>\n <li>Address patient and durable power of attorney (DPOA) inquiries via telephone.</li>\n <li>Respond to patient or caregiver messages received via the Curana Patient Portal.</li>\n <li>Assist patients with scheduling follow-up appointments with Curana Providers or specialists.</li>\n</ul>\n<p> </p>\n<p>Provider Support</p>\n<ul>\n <li>Manage electronic health records (EHR) and ensure accurate and up-to-date patient records.</li>\n <li>Coordinate documents needed for review or signature by a provider.</li>\n</ul>\n<ul>\n <li>Facilitate provider orders and escalate findings.</li>\n</ul>\n<ul>\n <li>Support Provider scheduling.</li>\n <li>Assists with prior authorizations.</li>\n</ul>\n<ul>\n <li>Obtain patient records and diagnostic test results.</li>\n <li>Organizing telehealth consultations.</li>\n</ul>\n<p> </p>\n<p>Communication Support</p>\n<ul>\n <li>Collaborate with the Curana Interdisciplinary Care Team to ensure seamless communication within our health network.</li>\n <li>Other duties as assigned</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong><u>Required Education and Experience</u></strong></p>\n<ul>\n <li>Registered or Certified Medical Assistant.</li>\n <li>1+ years of experience working in a medical office, community engagement, or other related fields.</li>\n</ul>\n<p> </p>\n<p><strong><u>Required Skills</u></strong></p>\n<ul>\n <li>Ability to interpret medical records, lab results, and appointment notes.</li>\n <li>Demonstrates experience in collecting clinical information and surfacing changes to the appropriate Curana care team member.</li>\n <li>Willingness to travel to local Senior Living Communities.</li>\n</ul>\n<ul>\n <li>Proficient computer skills and exhibits knowledge in using various Microsoft Office applications.</li>\n</ul>\n<ul>\n <li>Excellent organizational and time management skills with the ability to prioritize tasks.</li>\n</ul>\n<p> </p>\n<p><strong><u>Preferred Education and Experience </u></strong> </p>\n<ul>\n <li>Experience working with chronic care management (CCM) or other care management programs.</li>\n</ul>\n<p> </p>\n<p><strong><u>Travel Requirements: </u></strong></p>\n<ul>\n <li>On-site at assigned Senior Living Communities 4 business days out of the work week.</li>\n <li>May require travel to multiple local Senior Living Communities.</li>\n</ul>\n<p>This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required. If so, you will be provided with thirty (30) days notice of this requirement. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, Curana Health offers benefits such as, a comprehensive benefits package, 401K, PTO, paid holidays (all benefits are subject to eligibility requirements).",
