Springfield Memorial is seeking a dynamic physical therapist to join our outpatient team.</p>\n<p>In this role you will be responsible for evaluating and treating patients with a wide variety of impairments, working collaboratively with our physician partners, and advancing your skill set with our clinical ladder program.</p>\n<p> </p>\n<p>As a Physical Therapist, you will be responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines. </p>\n<p> </p>\n<p>Bachelor’s degree in Physical Therapy or entry level equivalent is required.  Must be Licensed or license pending in the State of Illinois and without previous disciplinary action in Illinois or any other state.  Current CPR card.</p>\n<p> </p>\n<p>Schedule</p>\n<p>Full Time, Days</p>\n<p>$30,000 Sign-On Bonus </p>\n<p>Contact me if interested at [email protected] or schedule a call with me to discuss at</p>\n<p>  https://MemorialHealthIL.as.me/KariWatson</p>\n<p> </p>\n<h2>Qualifications</h2>\n<p>Education:</p>\n<ul>\n <li>Graduate of an approved school of Physical Therapy.</li>\n <li>P.T. license in the State of Illinois, or license pending with a valid Authorization to Test letter (ATT) from the Federation of State Boards of Physical Therapy (FSBPT), is required.  The maximum allowable time to practice in a license pending status/under a licensed Physical Therapist is 6 months per licensing regulations.</li>\n</ul>\n<ul>\n <li>Basic knowledge in physical therapy practices in accordance with the P.T. Practice Act.</li>\n</ul>\n<p>Licensure/Certification/Registry:</p>\n<ul>\n <li>Illinois eligible or licensed Physical Therapist.</li>\n <li>CPR certification is required. </li>\n</ul>\n<h2>Responsibilities</h2>\n<p>Other Knowledge/Skills/Abilities:</p>\n<ul>\n <li>Universal Precautions Category I.</li>\n <li>Must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance.</li>\n</ul>\n<ul>\n <li>Provides rehabilitative care to patients of all ages. </li>\n <li>Evaluates treatment needs of assigned physician referred patients of all ages </li>\n <li>Implements the patient treatment according to the program plan, assesses the patient’s response and revised the program as needed. </li>\n <li>Effectively communicates with the patient, family members, hospital staff and the public. </li>\n <li>Keeps timely and accurate medical, administrative and education records. </li>\n <li>Coordinates effective patient care with the assistance of the Physical Therapy Assistant and Physical Therapy Aide. </li>\n <li>Maintains skills in treatment procedure through utilization of in-service and continuing education opportunities and expertise of other clinical staff personnel. </li>\n <li>Demonstrates effective use of departmental equipment for patient care </li>\n <li>Contributes to the maintenance of a satisfactory work environment. </li>\n <li>Promotes health and safety in the work environment. </li>\n <li>Actively participates in competency assessment process. </li>\n <li>Performs other related work as required or requested. </li>\n</ul>\n<p> </p>\n<p>The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job.
