    "description": "<p><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"font-weight: bold\">Wabanaki Public Health &amp; Wellness (WPHW) is growing, and we are excited to be adding new people to our team!</span>  If you value inclusivity, balance, and cultural centeredness and have a true passion for serving others, you may be a great fit for our team!  <br></span></p>\n<p><br></p>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt; font-weight: bold\"><span style=\"text-decoration: underline\">Position Summary</span>:</span></p>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt\">The Sage Way Peer Navigators are Certified Intentional Peer Support Specialist and provide peer support through Meaningful Activities as well as though provision of educational activities focused on goal planning, self-management, problem solving skills and Vocational Preparedness. Navigators assist participants other supportive services such as housing, food, health care, and wellness; and provide information, trainings, and technical assistance to the community, law enforcement, and providers.  </span></p>\n<p><br></p>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt; font-weight: bold\"><span style=\"text-decoration: underline\">Duties and Responsibilities</span>:</span></p>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"font-weight: bold\">Program/Project Management Support</span><br></span></p>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Provide outreach services to locate and connect unduplicated individuals, quarterly.  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Offer linkage(s) to community resources, treatment, and wellness programs, as identified and requested by the Participant, which must include but is not limited to:   </span></li>\n</ul>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Treatment for mental health and/or substance use, including Medication Assisted Treatment (MAT), as applicable.  </span></li>\n</ul>\n</li>\n</ul>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Detox and/or rehabilitation services.  </span></li>\n</ul>\n</li>\n</ul>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Overdose Prevention Through Intensive Outreach Naloxone and Safety (OPTIONS); </span></li>\n</ul>\n</li>\n</ul>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Medical care.  </span></li>\n</ul>\n</li>\n</ul>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">General wellness and/or Recovery services.  </span></li>\n<li><span style=\"font-family: georgia, palatino\">and Legal aid services, as applicable. <span style=\"font-size: 12pt\"> </span></span></li>\n</ul>\n</li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Support Participants in navigating challenges to access services, as needed and appropriate. </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Provide support and services in alignment with multiple recovery modalities.   </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Provide Participants with education and information about community resources, including treatment and Primary, Secondary and Tertiary Prevention services.  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Provide follow-up contact with Participants within thirty (30) calendar days of the initial contact, as able and consented to by the Participant.   </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Provide support to Participants with accessing treatment and/or Recovery support(s).  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">If the Participant indicates they no longer want to continue services with a current community provider, the Peer Navigator shall support the Participant in identifying other Participant directed options.  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">If the Participant indicates they no longer are interested in working with a new community provider, the Peer Navigator shall continue to provide Peer Support and outreach as desired by the Participant.  </span></li>\n</ul>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"font-weight: bold\">Information Preparation</span><br></span></p>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Promote and disseminate a calendar of services offered including but not limited to Peer Lead Support groups, Community and Stakeholder Trainings, and other related activities. </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Collaborate with Prevention staff to create and disseminate overdose prevention information and materials.  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Promote and disseminate HIV/ HCV and other sexual health information.  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Promote and refer participants to other clinical services including but not limited to: Case Management; Wellness Programs; Maternal Child Health, General Medical Support, and other available services within the catchment area.  </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Ensure that the website has current and accurate information about the program </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Ensure the Wabanaki Careline has the correct program information for referrals.  </span></li>\n</ul>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"font-weight: bold\">Logistical and Event Coordination</span><br></span></p>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Assist and collaborate with health fair and other outreach events. </span></li>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Plan and implement special events as appropriate. </span></li>\n</ul>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"color: #222222\">Perform other duties as assigned.</span></span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt; font-weight: bold\"><span style=\"text-decoration: underline\">Education and Experience Required</span>:</span></p>\n<ul>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">CIPSS Certified or eligible  </span></li>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Certified Recovery Coach or eligible</span></li>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">2 years of providing Substance use and Mental Health Related Services<br></span></li>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Familiar with multiple paths to recovery<br></span></li>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\">Lived experience preferred / one or more years of recovery work </span></li>\n</ul>\n<p><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"color: #222222\">Excellent planning, time-management, analytical, and organizational skills  </span></span></li>\n<li><span style=\"font-family: georgia, palatino; font-size: 12pt\"><span style=\"color: #222222\">Proficient in Microsoft Office, including Outlook, Word, Excel, PowerPoint as well as Microsoft 365, Teams, and Zoom. Project Management software experience is preferred</span></span></li>\n<li><span style=\"color: rgb(45, 45, 45); font-family: georgia, palatino; font-size: 12pt\">Possession of a valid driver’s license and the ability to be insured under the Agency’s automobile insurance plan at the standard rate </span></li>\n<li><span style=\"font-family: georgia, palatino;
