    "requisitionDescription": "<div><div><p><strong>Job Purpose &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</strong></p><p>The Care Coordinator Supervisor is responsible for providing on-going clinical supervision to Care Coordinators.</p><p><strong>Qualifications &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</strong></p><ul class=\"decimal_type\"><li>Master&rsquo;s degree in a human services field; OR</li><li>Anyone meeting the qualifications of a Qualified Mental Health Professional (QMHP) (without a master&rsquo;s degree in a human service field) and three years of experience providing community support services to individuals with mental illness and/or substance use disorders in accordance with the community support key service functions specified in 9 CSR 30-3.157 Community Support in CSTAR and 9 CSR 10-4.047 Community Support in CPR. &nbsp;Experience is specific to the adult or child/youth/adolescent population the individual will supervise. See below for qualifications of QMHP.&nbsp;<ul><li>A QMHP is an individual with a master&rsquo;s or bachelor&rsquo;s degree in a human services field (non-licensed) from a college or university included in the U.S. Dept. Of Education&rsquo;s database of accredited schools and/or is certified/credentialed to provide services within their scope of practice, including-Behavior Analyst; Community Support Specialist; Community Support Supervisor; Occupational Therapist; Physical Therapist; Psychiatric Pharmacist; Psychosocial Rehabilitation (PSR) Supervisor; Qualified Addiction Professional (QAP); Registered Nurse; and Youth Behavioral Health Liaison.&nbsp;</li></ul></li></ul><p><strong>Core Job Duties&nbsp;</strong></p><ul><li>Complete comprehensive assessments, treatment plans, quarterly functional assessment update, crisis intervention.</li><li>Use management strategies that focus on person(s)-served outcomes, care coordination, collaboration and&nbsp;communication&nbsp;with&nbsp;other&nbsp;service&nbsp;providers&nbsp;both&nbsp;within&nbsp;and&nbsp;external&nbsp;to&nbsp;FCC&nbsp;Behavioral&nbsp;Health</li><li>Provide&nbsp;training/supervising&nbsp;of&nbsp;staff,&nbsp;guidance&nbsp;and&nbsp;feedback,&nbsp;field&nbsp;mentoring&nbsp;and&nbsp;oversight&nbsp;of&nbsp;services.</li><li>Monitor&nbsp;and&nbsp;review&nbsp;services,&nbsp;interventions&nbsp;and&nbsp;contacts&nbsp;with&nbsp;person(s)-served;&nbsp;evaluate&nbsp;effectiveness&nbsp;of&nbsp;services.</li><li>Be proactive in identifying program issues and work collaboratively with program leadership to develop solutions.</li><li>Assist the team in providing coverage (in any area of operation in which they are qualified to do so) to ensure the program functions effectively and without disruptions.</li><li>Adherence&nbsp;to&nbsp;all&nbsp;applicable&nbsp;evidence-based&nbsp;practice&nbsp;models.</li><li>Orally&nbsp;communicate&nbsp;information&nbsp;effectively&nbsp;and&nbsp;accurately.</li><li>Assure program operates within the constraints of the agency&rsquo;s certification (CARF, DMH) standards and strategic plan&nbsp;related&nbsp;to&nbsp;clinical&nbsp;care.</li><li>Abide&nbsp;by&nbsp;program&nbsp;and&nbsp;agency&nbsp;policies&nbsp;and&nbsp;procedures</li><li>Other job duties and special projects as assigned.</li></ul></div></div>\n",
