Fitness Program</li><li>Legal Assistance through ARAG</li><li>Optum Financial Service through ConnectYourCare</li><li>Benefit Advocacy Center through Gallagher</li></ul><p><strong>Description of Duties and Responsibilities</strong></p><p>▪ Conducts direct street outreach to individuals in need of addictions medication services.</p><p>▪ Establishes a respectful, trusting relationship with program participants.</p><p>▪ Assists participants to identify and build on their strengths and resiliencies.</p><p>▪ Assists participants in identifying basic needs.</p><p>▪ Provides referrals for medication assisted treatment of substance use disorders.</p><p>▪ Provides guidance and resources to help participants access healthcare, behavioral healthcare, public assistance, housing, nutritional assistance and health insurance.</p><p>▪ Provide training to program participants and distributes medication for opioid overdose reversal.</p><p>▪ Distributes harm reduction supplies, including: wound care kits, safer sex kits, sharps disposal and medication</p><p>disposal kits, substance supply test kits for fentanyl and other synthetic drugs.</p><p>▪ Escorts participants to clinic for assessment and same day medication initiation.</p><p>▪ Assists patients with medication access.</p><p>▪ Facilitates warm hand-offs with network service providers.</p><p>▪ Acts as advocate on behalf of program participants with professional staff and network service providers.</p><p>▪ Participates in team meetings and service planning conferences.</p><p>▪ Documents delivery of services in case record.</p><p>▪ Participates in in-service training according to program requirements.</p><p><br></p><p><strong>Qualifications for Position</strong></p><p>▪ New York State Certified Recovery Peer Advocate or Provisional CRPA preferred.</p><p>▪ CARC preferred.</p><p>▪ Lived experience in recovery from substance use disorder.</p><p>▪ 2 years of direct care work experience with individuals at risk for, or with a substance use disorder.</p><p><br></p><p><strong>Skills and Knowledge Required</strong></p><p>▪ Demonstrates effective communication skills; recognizes and uses person-centered language.</p><p>▪ Recognizes that there are multiple pathways to recovery/wellness.</p><p>▪ Practices a strengths-based approach to recovery/wellness.</p><p>▪ Proficient with Microsoft Office applications</p><p>▪ Bi-lingual English/Spanish preferred</p><p>▪ Ability to engage active substance users and their families</p><p><br></p><p><strong>Amount of travel and any other special conditions or requirements</strong></p><p>Frequent local travel required</p><p><br></p><p><em>Odyssey House is an equal opportunity employer maintaining a non-discriminatory policy on hiring of its personnel.
