We have many visits available in all areas of San Antonio and other surrounding towns. Prime Home Health can provide you with competitive rates for visits and OASIS tasks along with paid mileage with an experienced, supportive administrative team.  </span></p>\n<p><br></p>\n<p><span style=\"font-size: 14px; font-family: helvetica;\">You will be responsible for assessing patients' needs, developing care plans, coordinating with other healthcare professionals, and implementing appropriate interventions to promote optimal health and well-being. Your expertise, clinical judgment, and strong interpersonal skills will be crucial in delivering high-quality care and ensuring positive patient outcomes.</span></p>\n<p><br></p>\n<p><span style=\"font-size: 14px; font-family: helvetica; font-weight: bold;\">Responsibilities:</span></p>\n<p><br></p>\n<ol>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Patient Assessment and Care Planning:</span>\n<ul>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop individualized care plans.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Collaborate with patients, their families, and healthcare teams to establish goals and interventions to address patients' health conditions and promote self-management.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Regularly reassess patients' conditions, monitor vital signs, and evaluate the effectiveness of interventions, adjusting care plans as necessary.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Educate patients and their families on disease management, medication administration, and self-care techniques.</span></li>\n</ul>\n</li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Skilled Nursing Care:</span>\n<ul>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Provide skilled nursing care including wound care, medication management, IV therapy, catheter care, and other treatments as prescribed.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Administer medications and treatments accurately and safely, following established protocols and guidelines.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Perform skilled procedures such as venipuncture, tracheostomy care, and respiratory therapy.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Monitor and manage symptoms, pain, and comfort levels, implementing appropriate interventions to alleviate distress.</span></li>\n</ul>\n</li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Coordination and Collaboration:</span>\n<ul>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and implement comprehensive care plans.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Coordinate with the interdisciplinary team to ensure continuity of care and effective communication.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Communicate regularly with patients, families, and caregivers to provide updates on the patient's condition, progress, and any changes to the care plan.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Coordinate and supervise home health aides and other support staff involved in patient care.</span></li>\n</ul>\n</li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Documentation and Compliance:</span>\n<ul>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Maintain timely, accurate and thorough documentation of assessments, care provided, and patient outcomes in Kinnser/Wellsky (EHR).</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Ensure compliance with all regulatory requirements, agency policies, and standards of care.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Complete and submit clinical documentation, including OASIS assessments, within specified timeframes.</span></li>\n</ul>\n</li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Quality Improvement:</span>\n<ul>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Participate in quality improvement activities, including case conferences, chart reviews, and performance improvement initiatives.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Identify areas for improvement in care delivery and actively contribute to the development and implementation of best practices.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Stay updated with the latest evidence-based practices and industry trends related to home health nursing.</span></li>\n</ul>\n</li>\n</ol>\n<p><span style=\"font-size: 14px; font-family: helvetica; font-weight: bold;\">Requirements:</span></p>\n<p><br></p>\n<ul>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Current and unrestricted Registered Nurse (RN) license valid in the State of Texas. </span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Minimum of one year of nursing experience in adult home health. </span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Experienced in all OASIS tasks: SOC, ROC, Recertification, Discharge and regular visits. </span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Experience with Kinnser/Wellsky a plus, additional training on Kinnser can be provided by our team. </span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Strong clinical assessment and critical thinking skills to effectively address patients' complex needs.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Ability to work autonomously, prioritize tasks, and manage time efficiently in a dynamic home health environment.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">Willingness to travel to patients' homes within the designated service area and have reliable transportation.</span></li>\n<li><span style=\"font-size: 14px; font-family: helvetica;\">BLS (Basic Life Support) certification.</span></li>\n</ul>\n<p><span style=\"font-size: 14px;
