These include the Beacon Award of Excellence at both Iowa Methodist Medical Center and Iowa Lutheran Hospital and the Best Hospital/Healthcare System and Best Company to Work For by readers of the Des Moines Business Record.</p>\n<p> </p>\n<p>Our growing Liver Clinic is seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner) to join our multidisciplinary hepatology team. This role provides comprehensive care for patients with a wide range of liver conditions, including cirrhosis, hepatitis, fatty liver disease, autoimmune liver disorders, pre- and post-transplant care, and other complex hepatology needs. The APP will work collaboratively with hepatologists, nursing, and support staff to deliver high-quality, patient-centered care. With the goal after training to see 12-15 patients per day, pending complexity.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Monday-Friday 8-4:30</li>\n <li>Conduct new and follow-up patient visits, including assessments, diagnosis, treatment planning, and ongoing management of liver-related conditions.</li>\n <li>Coordinate care for high-acuity and chronically ill patients, ensuring seamless communication across the care team.</li>\n <li>Order and interpret diagnostic tests, labs, imaging, and specialty evaluations.</li>\n <li>Manage medication adjustments, including hepatology-specific therapies and monitoring for side effects.</li>\n <li>Provide patient and family education related to liver disease, treatment plans, lifestyle interventions, and long-term care needs.</li>\n <li>Participate in care pathways for cirrhosis management, transplant evaluations, and post-transplant follow-up as applicable.</li>\n <li>Collaborate with inpatient teams to support transitions of care and reduce readmissions.</li>\n <li>Document all patient interactions in the electronic medical record accurately and in a timely manner.</li>\n <li>Contribute to quality improvement initiatives, care standardization, and process development within the clinic.</li>\n <li>Outreach clinic at least once monthly and could involve more pending volumes.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Licensed Nurse Practitioner (NP) in the state of Iowa</li>\n <li>Minimum of 2 years of experience in GI/hepatology, internal medicine, or a related specialty preferred.</li>\n <li>Demonstrated ability to manage complex patients in an outpatient specialty setting.</li>\n <li>Strong communication, critical-thinking, and patient education skills.</li>\n <li>Ability to work effectively in a multidisciplinary team.</li>\n <li>EMR proficiency (Epic preferred).</li>\n</ul>\n<p><strong>We offer</strong></p>\n<ul>\n <li>Attractive incentive package </li>\n <li>Shift and weekend differential pay </li>\n <li>A competitive Total Rewards program offers benefits that align with your needs and priorities, no matter what stage of life you are in. </li>\n <li>Generous time off and CME allowance, malpractice insurance, retirement contributions and more </li>\n <li>Dedicated provider resources to positively impact providers work-life balance</li>\n <li>Eligibility for Public Service Loan Forgiveness, including a free third-party service for loan restructuring and consolidation. </li>\n <li>A commitment to ensuring you have a voice that is heard regardless of role, race, gender, religion, or sexual orientation. </li>\n <li>Support and development opportunities that are a crucial part of delivering a remarkable employment experience. </li>\n <li>A culture guided by uncompromising values and unwavering belief in doing what’s right for the people we serve.</li>\n</ul>\n<p><strong>Community Details</strong></p>\n<p>Greater Des Moines (DSM) is a community that invests in itself, that is powered by multi-national corporations and also supports small, locally owned businesses.
