The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.</p>\r\n\r\n<p><strong><span style=\"font-size:14px;\">Essential Functions and Skills</span></strong><br />\r\n<strong>Patient Care</strong></p>\r\n\r\n<ul>\r\n\t<li>Provides care in accordance with the Nurse Practice Act and professional nursing standards.</li>\r\n\t<li>Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history.</li>\r\n\t<li>Develops, implements, and revises the Plan of Care based on ongoing patient evaluations.</li>\r\n\t<li>Administers medications and treatments as prescribed in the physician&rsquo;s plan of care.</li>\r\n\t<li>Educates patients and families on self-care, medications, and diet as appropriate.</li>\r\n\t<li>Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition.</li>\r\n\t<li>Coordinates care with physicians, interdisciplinary teams, and community health partners.</li>\r\n\t<li>Ensures a safe environment for patients and supports their activities of daily living.</li>\r\n\t<li>Provides emotional support and counseling to patients and families.</li>\r\n\t<li>Arrange for necessary medical equipment and supplies.</li>\r\n\t<li>Ensures HIPAA compliance and patient confidentiality.</li>\r\n</ul>\r\n\r\n<p><strong>Additional Duties</strong></p>\r\n\r\n<ul>\r\n\t<li>Participates in on-call visits as required including evenings and weekends.</li>\r\n\t<li>Attend interdisciplinary group meetings.</li>\r\n\t<li>Supervises ancillary personnel and delegates responsibilities as needed.</li>\r\n\t<li>Engage in professional development through training and continuing education.</li>\r\n\t<li>Contributes to quality assessment and performance improvement activities.</li>\r\n\t<li>Performs other duties as assigned.</li>\r\n</ul>\r\n",
