***Grant Funded Position*** Title: Psychiatric Mental Health Nurse Practitioner (PMHNP) Reports To: Chief Medical Officer Direct Reports: Students/Interns as appropriate Classification/Category: Clinical Level II Workgroup(s) : Provider Meeting Work Status : On Site Salary Range: Stipend(s): None Status: Exempt Summary of Position The Psychiatric Nurse Practitioner will provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders and health maintenance. This includes assessment, diagnosis, and management of mental health problems and psychiatric disorders. The Psychiatric Nurse Practitioner works collaboratively with other on-site medical and mental health providers. The Psychiatric Nurse Practitioner will model behaviors consistent with the published values and the Code of Conduct of RCCHC. RCCHC Mission, Vision and Values Mission : Great care for every one every day. Vision : Striving to be the primary healthcare provider, dedicated to and supported by the people we serve. Values : ( R ) espectful, ( C ) ompassionate, ( C ) aring, ( H ) onset, ( C ) omitted Population Served Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents within our service area and surrounding counties, regardless of their ability to pay. Minimum Qualifications Works effectively with others and is able to organize workload, as well as assume responsibility and maintain confidentiality. Must be able to work independently, exercise appropriate decision making skills, and function effectively on a team. Must be able to speak and communicate clearly and effectively. Must accept the several principles of community healthcare which include but are not limited to the following; commitment to serve patients regardless of their ability to pay, commitment to practices of health promotion and disease prevention, skill or aptitude in the management of physician and non-physician medical personnel. Education : A master's or doctoral degree from an accredited university or graduate program in psychiatric mental health nursing is required. Certification(s)/Licensure : Current unrestricted license as a Registered Nurse (RN) and as a Nurse Practitioner (NP) from the North Carolina (NC) Board of Nursing (BHON) is required. Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) by the American Nurses Credentialing Center (ANCC) is required. Current DEA licensure in NC is required. Experience: Three years of post-graduate clinical experience with psychiatric patients preferred but not required. Knowledge: Knowledge and thorough understanding of Federally Qualified Health Centers (FQHC’s) in regard to the Federal 330 Public Health Service Act. Knowledge and understanding of the 340B program. Knowledge of Electronic Medical Records (Epic) Knowledge of computer systems and applications. Skills: Skill in exercising a high degree of initiative, judgment, and discretion. Skill in analyzing situations accurately and taking effective action. Skill in establishing and maintaining effective working relationships with the medical and administrative staff, patients and the community. Skill in planning, organizing, and prioritizing work; delegating and achieving goals and objectives. Skill in exercising judgment and discretion in developing, interpreting, and implementing departmental policies and procedures. Skill in identifying and resolving problems. Skill in developing comprehensive reports. Abilities: Ability to plan, organize, and integrate priorities and deadlines. Ability to identify, analyze, and interpret complex data and, resolve operational problems. Ability to evaluate and make recommendations for continuous quality improvement. Ability to evaluate and make recommendations for continuous risk assessment. Ability to anticipate and react calmly in emergency situations. Ability to communicate clearly and effectively orally and in writing. Ability to competently use Microsoft Office, including Word, PowerPoint, Excel, and appropriate organizational software. Essential Functions Perform detailed psychiatric evaluations, including diagnostic assessments, psychiatric workups, and treatment planning, ensuring thorough and accurate documentation. Prescribe and manage medications in accordance with treatment guidelines, monitor patient responses, and adjust treatments as necessary. Work closely with other medical and behavioral health providers within and outside the organization to ensure integrated and comprehensive patient care. Provide education to patients and their families about mental health conditions, treatment options, and medication management, ensuring they understand the risks, benefits, and expected outcomes. Offer consultation services to other healthcare providers, contributing to the development of treatment plans and providing expert advice on psychiatric care. Order and interpret laboratory tests, explain results to patients, and take appropriate actions based on findings. Maintain accurate and timely documentation of all patient encounters, treatment plans, and follow-up care in compliance with organizational policies and procedures. Adhere to all performance standards set by the organization and funding agencies, participating in quality improvement initiatives and continuing medical education activities. As needed, may be asked to serve on committees focused on clinical practice improvement and performance assurance, contributing to the development and implementation of best practices. Anticipate and respond calmly to emergency situations, providing appropriate interventions and support. Engage in follow-up care with patients, ensuring continuity of care and addressing any ongoing or emerging mental health needs Maintain compliance with workflows, policies, and procedures to support the Patient-Centered Medical Home (PCMH) model of care and accreditation standards. Perform other duties as assigned, which may include participating in community service programs, health fairs, and educational presentations. Attend medical staff meetings and participate in CME activities. Collaborat e with the supervising physician, the nursing staff, technical personal, Behavioral Health providers and other Behavioral Health staff within RCCHC and external sources. Provides psychiatric health services education, counseling and emotional support. Refers patients as appropriate for inpatient care. May also provide primary medical care services as indicated, in accordance with statues, regulations and protocols regulating the profession. Performs other duties as assigned. Other job duties may be required that are not listed above.
