    "description": "<p><span style=\"font-size: 12pt\">The Clinical Care Coordinator is a licensed healthcare professional and a vital member of a multidisciplinary team, providing comprehensive, client- and family-centered care management services. This role supports individuals with disabilities and complex healthcare needs through individualized plans of care that promote care coordination, continuity, quality outcomes, and community inclusion. Guided by Medicalincs’ mission and core values, the Clinical Care Coordinator manages an assigned caseload in accordance with the Case Management Society of America (CMSA) definition of case management and ethical standards. The position might include monthly in-person visits across Maryland in client homes, nursing facilities, schools, and other residential or care settings, as well as regular collaboration with interdisciplinary team members.</span></p>\n<p><br></p>\n<p><span style=\"font-size: 14pt; font-weight: bold\">Key Responsibilities &amp; Competencies</span></p>\n<p><br><br></p>\n<ul>\n<li><span style=\"font-size: 12pt\">Facilitate admission to a discharge from care management services</span></li>\n<li><span style=\"font-size: 12pt\">Apply the case management process to guide service delivery</span></li>\n<li><span style=\"font-size: 12pt\">Develop and maintain individualized plans of care, including desired outcomes</span></li>\n<li><span style=\"font-size: 12pt\">Coordinate services and supports to ensure plan implementation and compliance with contractual and regulatory requirements</span></li>\n<li><span style=\"font-size: 12pt\">Monitor ongoing services, utilization, and cost-effectiveness; recommend plan modifications as needed</span></li>\n<li><span style=\"font-size: 12pt\">Evaluate desired versus actual outcomes and document progress</span></li>\n<li><span style=\"font-size: 12pt\">Identify and recommend cost-effective alternatives to care</span></li>\n<li><span style=\"font-size: 12pt\">Serve as a clinical resource to community service providers</span></li>\n<li><span style=\"font-size: 12pt\">Maintain ongoing communication with clients, families, and healthcare team members</span></li>\n<li><span style=\"font-size: 12pt\">Convene, attend, and actively participate in multidisciplinary team meetings</span></li>\n<li><span style=\"font-size: 12pt\">Deliver services through a combination of on-site visits and virtual/remote engagements</span></li>\n<li><span style=\"font-size: 12pt\">Maintain accurate, timely, and complete care management records</span></li>\n<li><span style=\"font-size: 12pt\">Document client interactions in the client database within 48 hours of contact</span></li>\n<li><span style=\"font-size: 12pt\">Ensure compliance with HIPAA and all applicable professional standards</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 14pt; font-weight: bold\">Qualifications</span></p>\n<p><br><br></p>\n<ul>\n<li><span style=\"font-size: 12pt\">Bachelor of Science in Nursing (BSN) with active Registered Nurse (RN) license or</span></li>\n<li><span style=\"font-size: 12pt\">Master's degree in social work (MSW) with active LMSW or LCSW-C license</span></li>\n<li><span style=\"font-size: 12pt\">Degree must be from an accredited college or university or equivalent education and experience</span></li>\n<li><span style=\"font-size: 12pt\">CCM (Certified Case Manager) certification preferred; required within two years of hire</span></li>\n<li><span style=\"font-size: 12pt\">Minimum three (3) years of professional experience in one or more of the following: Pediatric intensive care nursing, Ambulatory health care, Rehabilitation nursing, Clinical social work in a community-based setting with individuals with complex healthcare needs</span></li>\n<li><span style=\"font-size: 12pt\">Demonstrated clinical competence and ability to work independently with minimal supervision</span></li>\n<li><span style=\"font-size: 12pt\">Active state licensure and current malpractice insurance (maintained at all times)</span></li>\n<li><span style=\"font-size: 12pt\">Maintenance of applicable professional certifications (e.g., CCM, RN, LCSW-C)</span></li>\n<li><span style=\"font-size: 12pt\">Valid driver's license, reliable transportation, and proof of automobile insurance</span></li>\n<li><span style=\"font-size: 12pt\">Ability to travel throughout Maryland</span></li>\n<li><span style=\"font-size: 12pt\">Designated home office meeting all HIPAA requirements</span></li>\n<li><span style=\"font-size: 12pt\">Ability to telework from a home-based office</span></li>\n<li><span style=\"font-size: 12pt\">High-speed internet access</span></li>\n<li><span style=\"font-size: 12pt\">Proficiency with Microsoft Office Suite (Word, Excel, Access, Outlook), databases, and remote communication/documentation tools</span></li>\n<li><span style=\"font-size: 12pt\">Excellent organizational, prioritization, and interpersonal skills</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 14pt;
