Become a part of our specialized care staff and help us transform lives together.</p>\n<p> </p>\n<p></p>\n<p> </p>\n<p> </p>\n<h2>Job Overview</h2>\n<p>The Social Worker position requires understanding of common ethical and legal issues in social work practice in health care settings including: biopsychosocial–spiritual assessment; use of the strengths perspective; client and family engagement in all aspects of social work intervention; case management/care management/care coordination/health care navigation; discharge and transition planning; client concordance with and adherence to the plan of care; Advance Care Planning, Palliative Care, including pain and symptom management, Hospice and end-of-life care; identification of child abuse, trauma, neglect, and exploitation; crisis intervention, facilitation of benefits and resource acquisition to assist clients and families, including an understanding of related policies, eligibility requirements, and financial and legal issues; advocacy with other members of the interdisciplinary team and within the health care institution to promote clients’ and families’ decision making and quality of life; client, family, interdisciplinary, and community education; family systems issues, including the impact of health care concerns, illness, and disease on family relationships; life cycles, and care-giving roles and support needs. The pay range for this position is $70,054- $105,081 annually.  Compensation is determined based on years of experience and departmental equity.  </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.</li>\n <li>Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.  </li>\n <li>Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.</li>\n <li>Collaborates by working with patients, families, caregivers and the health care team locally and within the system to jointly communicate, problem solve, and share accountability for optimal outcomes.</li>\n <li>Arranges services among community agencies, physicians, patients, family, caregivers and others involved in the plan of care.</li>\n</ul>\n<p>Note:  This is not an all-inclusive list of this job's responsibilities.  The incumbent may be required to perform other related duties and participate in special projects as assigned.</p>\n<h2>Qualifications</h2>\n<p><strong>Required</strong></p>\n<ul>\n <li>Bachelor in Social Work </li>\n <li>SW license in state of practice</li>\n</ul>\n<p><strong>Preferred</strong></p>\n<ul>\n <li>Master's in Social Work</li>\n <li>3 years Social Work </li>\n <li>Case Management Certification (CMC, ACM or ANCC)</li>\n <li>EMR (Epic) experience</li>\n</ul>\n<p> </p>",
