The MST Coach is a specialized Case Planner role with a concentration of the MST-FIT model of service. Working within the social service department, the Coach supports youth and families, along with department staff by wrapping services to ensure successful discharge.</p><br><strong><strong>MST-FIT </strong>(<em>Multi-systemic Therapy/Family Integrated Transition</em><strong>) Coach</strong></strong><p>Perform case management responsibility to the youth using the MST approach. Performs all activities and administrative tasks associated with handling a caseload in keeping with all ACS and OCFS regulations. Field work that sometimes requires you to drive residents in your vehicle. Occasional evening hours are required for parent conferences and home visits. </p><p>Our main campus in Rockaway Park, Queens, houses a Residential Treatment Center, Close to Home Non-Secure Placement programs, administrative offices, clinical services, and an on-site NYC Department of Education school.
