Description About Us The Children's Guild Transformation Academy is part of The Children's Guild — a nonprofit organization dedicated to transforming how America educates and cares for its children. The Transformation Academy serves students with complex academic, behavioral, and social-emotional needs, providing a therapeutic, trauma-informed environment where every young person can achieve, excel, and succeed. Our continuum of care connects students to the supports they need — in school and beyond. The Opportunity We are seeking a dedicated and resourceful Transition Coordinator to support students in successfully navigating their transition from the Transformation Academy to post-secondary education, employment, independent living, and community engagement. The Transition Coordinator serves as a vital bridge between the school, families, agencies, and the broader community — ensuring that every student leaves with a clear, supported pathway forward. Key Responsibilities Transition Planning & IEP Support Develop, coordinate, and implement individualized transition plans in alignment with each student's IEP goals, strengths, interests, and post-secondary goals Participate actively in IEP meetings to ensure transition goals are meaningful, measurable, and student-centered Ensure compliance with IDEA transition requirements including age-appropriate transition assessments, measurable post-secondary goals, and coordinated transition services Maintain accurate and timely transition-related documentation for all eligible students Coordinate with special education staff, teachers, and clinicians to align transition services with academic and therapeutic programming Student Support & Career Development Conduct individual transition assessments to identify each student's interests, strengths, and post-secondary goals Provide direct instruction and support in areas including career exploration, job readiness, self-advocacy, independent living skills, and community integration Connect students to vocational training, internship, apprenticeship, and employment opportunities Support students in applying to post-secondary education programs, vocational schools, and workforce development programs Coach students on resume writing, interview skills, workplace expectations, and financial literacy Agency & Community Partnerships Build and maintain relationships with community partners including employers, vocational rehabilitation agencies, workforce development programs, higher education institutions, and social service providers Coordinate referrals to the Division of Rehabilitation Services (DORS), Department of Developmental Disabilities (DDA), and other relevant agencies Connect students and families to community resources that support independent living, housing, transportation, and healthcare Represent the Transformation Academy at community transition fairs, interagency meetings, and partnership events Family Engagement Partner with families and caregivers as active participants in the transition planning process Educate families on transition services, post-secondary options, and community resources available to their students Maintain open, consistent, and culturally responsive communication with families throughout the transition process Support families in understanding students' rights under IDEA and self-advocacy principles Data, Compliance & Reporting Maintain up-to-date transition records and documentation in compliance with IDEA, MSDE, and agency requirements Track student transition outcomes including post-secondary placement, employment, and community participation Prepare and submit required transition-related reports to school leadership and regulatory bodies Monitor and report on the progress of transition goals at regular intervals throughout the school year Collaboration & Professional Development Serve as a transition resource and consultant to teachers, clinicians, and school staff Participate in multidisciplinary team meetings, staff meetings, and school-wide professional development Stay current on transition best practices, legislation, and community resources Attend agency and community-based training and professional development opportunities Qualifications Education and/or Experience Minimum requirement of a Bachelor’s Degree in special education, guidance counseling, or a related field and a minimum of two years working in transition services or special education. Certificates, Licenses, Registrations Certified or be eligible for certification in crisis management. Must possess a valid Maryland State driver’s license. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
