Perform duties while actively promoting an environment consistent with MHA’s Cultural Equity Policy and WELCOME Orange.</p><p><strong>Responsibilities:</strong></p><ul><li>Attend bi-weekly Care Coordination team meetings.</li><li>Attend trainings and meetings relevant to AOT Care Management.</li><li>Will assist clients in becoming effectively and actively engaged in managing their healthcare to improve individual health outcomes while reducing avoidable hospitalizations and emergency department utilizations.</li><li>Conduct four monthly in person visits with clients in the community</li><li>Complete New York State Community Mental Health (CMH) assessments and reassessments.</li><li>Will provide leadership and support by facilitating and leading multidisciplinary teams of medical, mental health, chemical dependency treatment providers, courts/legal entities, social workers, nurses, representatives of the Local Government Unit (LGU) as directed by the county AOT Coordinator and other care providers toward assuring enrollees receive services as indicated in the enrollee’s single plan of care.</li><li>Submit monthly billing and weekly case reports for caseload.</li><li>Complete Care Plans, weekly status reports, and other monthly data collection information.</li><li>Attend monthly Orange County AOT/Forensic Care Management</li><li>Provide 24-hour on-call services when scheduled.</li><li>Maintain all client records electronically through Foothold Technology and CAIRS Electronic Record Keeping.</li><li>Maintain all client files according to program policies and procedures.</li><li>Establish and maintain linkage with appropriate community agencies and service providers.</li><li>Represent MHA at meetings as necessary.</li></ul><p><strong>Other Duties:</strong></p><ul><li>Participate in MHA’s committees as needed.&nbsp;</li><li>The duties outlined above are intended to describe the essential job functions, the general supplemental functions and the essential requirements for the performance of this job.
