We also provide attractive working conditions and opportunities for career growth through professional development.</p>\n<p> </p>\n<p><strong>Discover why Cooper University Health Care is the employer of choice in South Jersey.</strong></p>\n<h2>Short Description</h2>\n<ul>\n <li>Bridge the gap between clinician and patient, by providing individual support and assistance in navigating the health care system to provide continuity of care.</li>\n <li>Working with current RN Navigator disease team to provide non-clinical support regarding    scheduling, pre cert process, follow up calls and record retrieval for timely appointments.</li>\n <li>Call new patients within 24 hours of visit to CCI to check in regarding follow up appointments and needs.</li>\n <li>Work with CPN, Navigators, Social Work, Dieticians, Call Center, Providers, Radiology scheduling, chemo scheduling, and phone triage to support patient's needs.</li>\n <li>Participate in daily multi-disciplinary rounding on inpatient oncology unit, and identify needs and opportunities for coordination of care, follow up and scheduling.</li>\n <li>Act as a member of the Transition of Care team assisting patients in transitioning from inpatient to outpatient areas without interruption in services and care.</li>\n <li>Assist in connecting patients to services for support (ie: social work, financial counseling).</li>\n <li>Tracking of all patient contacts, follow ups, barriers to care and outcomes.</li>\n <li>Ensures the delivery of patient care through the coordination of customer services</li>\n <li>Participates in interdepartmental and ancillary activities to ensure quality, cost-effective patient care.</li>\n <li>Responsible for all core competencies and Cooper standards of care</li>\n</ul>\n<h2>Experience Required</h2>\n<ul>\n <li>Experience working in a healthcare office setting, or community outreach and education required. </li>\n <li>Knowledge of health care, inpatient/outpatient settings, Oncology specific patient population</li>\n <li>Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.</li>\n <li>Ability to take direction, follows through with tasks, and work independently.</li>\n <li>Flexibility, optimism, and a cooperative working style.</li>\n</ul>\n<h2>Education Requirements</h2>\n<ul>\n <li>High School Diploma or Equivalent; clinical training (i.e. MA, CNA, LPN) a plus</li>\n</ul>\n<h2>Salary Min ($)</h2>USD $21.00\n<h2>Salary Max ($)</h2>USD $33.00",
