This role supports a team-based care model and helps guide daily clinical workflows to ensure patients receive high-quality, efficient care.</p>\n\n<p style=\"text-align:justify\">The ideal candidate is a strong communicator who can adapt quickly in a busy clinical environment while delivering safe, compassionate nursing care.</p>\n\n<hr />\n<h2><strong>Key Responsibilities</strong></h2>\n\n<h3><strong>Patient Care &amp; Clinical Support</strong></h3>\n\n<ul>\n\t<li>Provide direct patient care within the RN scope of practice in an outpatient clinic setting</li>\n\t<li>Perform patient assessments, vital signs, and patient intake</li>\n\t<li>Assist providers with examinations, procedures, and treatments</li>\n\t<li>Administer medications, injections, and immunizations as ordered</li>\n\t<li>Perform clinic-based procedures including dressing changes, EKGs, nebulizer treatments, and suture or staple removal</li>\n</ul>\n\n<h3><strong>Care Coordination &amp; Triage</strong></h3>\n\n<ul>\n\t<li>Triage patient phone calls and portal messages using established protocols</li>\n\t<li>Coordinate care between providers, clinic staff, and patients</li>\n\t<li>Assist with medication refills and prior authorizations following clinic protocols</li>\n\t<li>Ensure appropriate follow-up care and appointment scheduling</li>\n</ul>\n\n<h3><strong>Patient Education &amp; Support</strong></h3>\n\n<ul>\n\t<li>Provide education to patients and families regarding conditions, medications, and treatment plans</li>\n\t<li>Promote preventative health and wellness education</li>\n\t<li>Reinforce provider instructions and care plans</li>\n</ul>\n\n<h3><strong>Documentation &amp; Quality Compliance</strong></h3>\n\n<ul>\n\t<li>Document patient care accurately in the electronic health record</li>\n\t<li>Assist with collection of quality and performance data</li>\n\t<li>Maintain compliance with regulatory standards and clinic protocols</li>\n\t<li>Participate in quality improvement initiatives</li>\n</ul>\n\n<h3><strong>Team Collaboration</strong></h3>\n\n<ul>\n\t<li>Support and mentor clinical support staff including LPNs and Medical Assistants</li>\n\t<li>Assist with training new clinical staff members</li>\n\t<li>Work collaboratively with the clinic team to improve patient experience and clinic operations</li>\n</ul>\n\n<hr />\n<h2><strong>Join a Hospital That Feels Like Family &ndash; In a Town That Feels Like Home&nbsp;</strong></h2>\n\n<p>Pagosa Springs Medical Center&nbsp;is an award-winning, independent Critical Access Hospital serving the residents and visitors of Archuleta County and the surrounding region.
