We visit approximately 80,000 patients each year in homes, schools, clinics, workplaces and retirement homes, spread across 14 branches and 21 nursing clinics.</p>\n<p> </p>\n<p> </p>\n<h2>Overview</h2>\n<p>We are currently looking for someone to join our dynamic Nursing Team in the role of <strong>Clinical Practice Lead. </strong>This person will work closely with our provincial Professional Practice Research & Education (PPRE) team and support clinical standards, orientation, and training. They will collaborate with leaders and help align strategic goals, priorities and initiatives set out and defined by our unique patient populations and Ontario Health Teams (OHTs).</p>\n<p> </p>\n<p><strong>We are looking to fill a Temporary 6-month Full-Time Clinical Practice Lead position in the Southwest - Owen Sound Branch, must be able to work in Grey, Bruce counties. </strong></p>\n<p> </p>\n<p><strong>Participation in the Branch On-Call schedule is required, which includes evenings and weekends on a rotational basis.</strong></p>\n<h2>What We Offer</h2>\n<ul>\n <li>Competitive salary, comprehensive health and dental benefits</li>\n <li>Other employment perks such as Employee Assistance Program, Perkopolis, Rewards Points</li>\n <li>Flexible work from home arrangements supporting employee work life balance</li>\n <li>Inspiring leadership and opportunities for professional growth</li>\n <li>Supportive & dedicated Safety Health & Wellness team & Pandemic Response team</li>\n <li>Interprofessional collaboration with our Professional Practice Research & Education Team</li>\n <li>This position offers a competitive compensation grid, with salary progression based on hours worked and role-specific criteria. <strong>The salary range is $79,170.00 to $99,528.00</strong>, and placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.</li>\n</ul>\n<h2>What The Role Involves</h2>\n<ul>\n <li>Supports, mentors and coaches front line clinical staff while delivering ongoing clinical education designed by the PPRE team.</li>\n <li>Works with regional management with respect to Community Health Services, in cooperation with the Ontario Health atHome and OHT if applicable.</li>\n <li>Approves, trains and monitors special functions as outlined in the Regulated Health Professions Act and in compliance with Community Health Services policies and practices</li>\n <li>Responsible for maintaining quality management procedures and practices within the Nursing team and department. </li>\n <li>Promotes and markets Community Health Services and maintains positive public relations.</li>\n <li>Builds relationships to enhance local capacity and understanding of system navigation supports, at a local and regional level, to support holistic patient care delivery</li>\n <li>Uses new technology e.g. virtual care to support frontline staff and patients to achieve desired care plan goals</li>\n <li>Promotes and provides active guidance for workplace Health and Safety.</li>\n <li>Performs other duties as required.</li>\n</ul>\n<h2>What You Bring</h2>\n<ul>\n <li>Regulated Health Professional <strong>Registered Nurse/ Registered Practical Nurse</strong></li>\n <li><strong>Registered with the CNO. </strong></li>\n <li>2+ years of community experience in leading exceptional clinical practice in a community care setting</li>\n <li>Experience in a formal leadership role considered an asset</li>\n <li>Strong palliative experience, an asset</li>\n <li>Wound care experience, an asset</li>\n <li>Knowledge of adult learning principles, an asset</li>\n <li>Strong proficiency in all aspects of Microsoft Office.</li>\n <li>Vehicle is required and some occasional travel within Ontario is required</li>\n <li>Demonstrated ability to handle highly sensitive and confidential information.</li>\n <li>Excellent English verbal and written communication skills</li>\n <li>French language skills are an asset</li>\n</ul>\n<h2>CarePartners In Your Community</h2>\n<p>In addition to providing home-based personal support, rehabilitation and therapy, nursing and palliative care across Ontario, CarePartners also serves the community through clinics, transitional care units, and provides relief in nursing homes and shared care settings.
