    "description": "<p><span>Service Type: Home Healthcare</span></p>\n<p><span>Patient Type: Medically Fragile Adult</span></p>\n<p><br></p>\n<p><span>Employment Type: Full-Time &amp; Part-Time Positions Available</span></p>\n<p><br></p>\n<p><span>The (LPN) Licensed Practical Nurse will provide care to clients according to their Plan of Care under the supervision of a Registered Nurse (“RN”) according state and federal rules and regulations and A New Well, Inc. philosophy, policies and procedures.</span></p>\n<p><br></p>\n<p><span>Services <em>may</em> include but are not limited to:</span></p>\n<p><br></p>\n<ul>\n<li>Tracheostomy Care<br></li>\n<li><span>Specialize Wound Care</span></li>\n<li><span>Administer Medications/Injections</span></li>\n<li><span>Measure In-take and Output</span></li>\n<li><span>Tube Feedings</span></li>\n<li><span>Vital Signs</span></li>\n<li><span>Nebulizer Treatment or MDI Puff Adm.</span></li>\n<li><span>Oxygen Administration via Tank/Concentrator</span></li>\n<li><span>Gastrostomy Care (G/J-Tube) &amp; Maintenance, Feeding &amp; Medication Administration</span></li>\n<li><span>Ventilator Alarms &amp; Function Checks</span></li>\n<li><span>Provide Activity of Daily Living</span></li>\n<li><span>Monitor Catheters</span></li>\n<li><span>Dress Wound</span></li>\n<li><span>Observe/Report Changes in client’s Condition</span></li>\n<li><span>Obtain Specimens for Analysis</span></li>\n<li><span>Teach Family/Responsible Party about client Care</span></li>\n<li><span>Light House Keeping</span></li>\n<li><span>Meal Preparations</span></li>\n<li><span>Evaluate the significance of assessment findings and communicates any pertinent information about the client’s status and ongoing needs to the Clinical Director/RN;</span></li>\n<li><span>Demonstrate knowledge of medications and their correct administration based on age of the client and client’s clinical position;</span></li>\n<li><span>Ability to perform wound care (Ostomy/Stoma Care, Ostomy Bag Change);</span></li>\n<li><span>Complete clinical records reviews, electronic progress note entries and documentation of supervisory visits to comply with Medicaid/Medicare policy standards;</span></li>\n<li><span>Perform all aspects of client care in an environment that optimizes the client’s safety and reduces the likelihood of medical/health care errors;</span></li>\n<li><span>Perform client care responsibilities considering needs to the standard of care for client’s age;</span></li>\n<li><span>Promote client/family/caregiver education using various verbal and written communication techniques that take into account the client’s/family’s cultural, ethnic and/or personal needs or preferences;</span></li>\n<li><span>Complete and submit necessary documentation in accordance state and federal rules and regulations and with Company’s policies and procedures;</span></li>\n<li><span>Maintains currency of professional knowledge by participating in continuing education;</span></li>\n<li><span>Participate in interdisciplinary team conferences in accordance with Company policy;</span></li>\n<li><span>Identify and assesses appropriate resources to meet client and/or family needs to facilitate optimal client outcomes;</span></li>\n</ul>\n<p><br></p>\n<p><span>Skills &amp; Qualifications</span></p>\n<p><br></p>\n<ul>\n<li>Excellent interpersonal skills, including customer service, patience, empathy and compassion;</li>\n<li>Effective communication skills, including active listening, writing, speaking and reading comprehension;</li>\n<li>Good collaboration and teamwork skills;</li>\n<li>Fast and adaptive problem-solving abilities;</li>\n<li>Keen attention to detail;</li>\n<li>In-depth understanding of patient care policies and regulations;</li>\n<li>Familiarity with symptoms and signs of improvement for a number of medical conditions;</li>\n<li>Comfortable working with specific populations of patients, including elderly patients and those with special needs;</li>\n<li>Physical strength and stamina;</li>\n</ul>\n<p><br></p>\n<p><span>Employment Requirements</span></p>\n<p><span><br>To ensure the safety and wellbeing of our employees, patients and communities we serve, certain positions within A New Well, Inc. may require evaluations, vaccinations, professional licensure and certifications.
