We’re seeking a dedicated per diem Physical Therapist to join our team in a supportive, energetic, and patient-focused setting. </p>\n<p> </p>\n<p>The Physical Therapist (PT) provides evaluation and therapy services, including direct patient care, patient/family education and transition of care, in an efficient, age-appropriate, and culturally sensitive manner upon licensed independent practitioner referral.  </p>\n<p> </p>\n<p><strong>Compensation: </strong> The pay range for this position is $35.66 hourly to $53.49 hourly. Compensation is determined based on years of experience and department equity.</p>\n<p> </p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Upon receiving a licensed independent practitioner referral, implements a systematic patient examination and evaluation utilizing comprehensive compliant and safe clinical care demonstrating the philosophy of family centered care</li>\n <li>Works with medical staff and the multidisciplinary team to design and implement individualized plans of care to meet the needs of the patient and their family.</li>\n <li>Makes appropriate discharge plans by collaborating with patients, families and the interdisciplinary team.</li>\n <li>Functions as a team member and develops leadership skills through activities such as mentoring and providing education to students and other professionals.</li>\n <li>Supports risk management and compliance initiatives through accurate, thorough documentation.</li>\n</ul>\n<p>This is not an all-inclusive list of this job’s responsibilities.
