Monthly internet/MIFI allowance Monthly cell phone allowance All Staff Benefits- IRA Plan with a 100% match on 3%, for any employee that works over $5,000 of income with our company A competitive employee referral program Toll reimbursement Mileage reimbursement at .50 a mile Employee recognition program Unlimited potential for growth An extensive and tailored employee orientation led by clinicians Job Summary As an Occupational Therapist (OT), you are key to helping us provide superior patient care. You will work to provide skilled Occupational Therapy services in accordance with the client’s plan of care to provide skilled occupational therapy service on an intermittent basis to clients in their homes in accordance with the physician's orders. We are looking for a team player who is eager to learn, passionate about providing quality care, and has a proven track record of compassionate care. What you will do in this role: Obtain a medical history, particularly as it relates to the present condition. Evaluate the client, the client’s significant other(s) and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other agency and community services will be required. Assure that the physician’s orders are appropriate and discuss necessary changes. Supervise the Occupational Therapy Assistant (COTA), as needed. Participate in agency activities, in‐services, and meetings, when appropriate. Confirm, on a weekly basis, the scheduling of visits with the Director of Nursing, case manager, and scheduler. Makes arrangements for outpatient services when procedures are required that cannot be given in the home. Evaluate the effectiveness of occupational therapy intervention and update the client care plan as needed. Prepare clinical/progress notes on the day of the visit which are incorporated into the chart within 72 hours. Instruct clients in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices. Teach, supervise and counsel the family and client in the total Occupational Therapy program and other related problems of the client at home. Establish goals based on the client's current level of functioning and potential for improvement and perform re‐evaluations as indicated. Qualifications Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association Preferably should be a member of the American Occupational Therapy Association Must be licensed/registered in Virginia Must have current CPR certification, or be able to obtain one before beginning You must be able to provide two references Must have at least one year of institutional/home health experience You must be able to pass OIG checks, Virginia State Police background check, and national background checks, and able to verify employment eligibility with necessary documentation Reliable transportation, proof of valid car insurance, and a valid driver’s license Commonwealth Home Care “The Standard of Care from People Who Care” We are an equal opportunity employer and value diversity at our company.
