We take pride in being a place employees want to work, as well as eagerly pursue quality outcomes for patients and families alike.</p><p><strong>How you’ll contribute:</strong></p><ul style=\"list-style-type: disc;\"><li>Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge</li><li>Creates and maintains up to date aftercare coordination community resources</li><li>Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.</li><li>Provides resource information to patients and families as appropriate for future care needs.</li><li>Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment.</li><li>Provides resource information to patients and families as appropriate for future care needs.</li><li>Creates a detailed Discharge Summary and Contract for each patient as assigned.</li><li>Completes all hospital and client documentation thoroughly, timely, and in accordance with hospital standards.</li><li>Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team when crisis occurs, assisting others to ensure patient safety.</li><li>Displays a thorough knowledge of confidentiality. Completes releases appropriately.
