How you’ll contribute: Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge Creates and maintains up to date aftercare coordination community resources Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist. Provides resource information to patients and families as appropriate for future care needs. Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients' treatment. Provides resource information to patients and families as appropriate for future care needs. Creates a detailed Discharge Summary and Contract for each patient as assigned. Completes all hospital and client documentation thoroughly, timely, and in accordance with hospital standards. Demonstrates appropriate crisis intervention and de-escalation skills. Acts as member of team when crisis occurs, assisting others to ensure patient safety. Displays a thorough knowledge of confidentiality. Completes releases appropriately. Demonstrates knowledge of HIPAA regulations and abides by those standards. Demonstrates a working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws.
