Our facility located in the South Hills of Pittsburgh, Pennsylvania, provides many levels of care.  Currently we are looking to hire a Discharge Planner.  </strong></p>\n<p><strong>please note: Southwood is not on the public transportation line.  </strong></p>\n<p><strong>PURPOSE STATEMENT:</strong> </p>\n<p>​Provide assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral, mental or health care follow up after discharge from the facility. </p>\n<h2>Responsibilities</h2>\n<p><strong>​</strong><strong>ESSENTIAL FUNCTIONS:</strong> </p>\n<ul>\n <li>​Develop continuing after care plan, based on patient's needs, to promote long term health and relapse prevention.   </li>\n <li>​Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.  </li>\n <li>​Make arrangements for community resources to facilitate discharge.  Provide all needed documentation to other providers to facilitate the continuum of care. </li>\n <li>​Act as a liaison between referral sources, patients and counselors. </li>\n <li>​Ensure all patient discharge planning documentation is clear, concise and timely.  </li>\n</ul>\n<p><strong>​</strong><strong>OTHER FUNCTIONS: </strong> </p>\n<ul>\n <li>​Perform other functions and tasks as assigned. </li>\n</ul>\n<p> </p>\n<p>SWOOD</p>\n<h2>Qualifications</h2>\n<p><strong>​</strong><strong>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</strong> </p>\n<ul>\n <li>​Associates degree in nursing or human services field required.  Bachelor's degree in social work or human services field preferred. </li>\n <li>​One or more years’ experience in a clinical setting preferred. </li>\n</ul>\n<ul>\n <li>​One or more years’ experience in discharge planning preferred. </li>\n</ul>\n<p><strong>​</strong><strong>LICENSES/DESIGNATIONS/CERTIFICATIONS: </strong> </p>\n<ul>\n <li>​If applicable, current licensure as a LPN, RN within the state where the facility provides services. </li>\n <li>​CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.   </li>\n <li>​First aid may be required based on state or facility requirements. </li>\n</ul>\n<p><strong>​</strong> </p>\n<p>We are committed to providing equal  employment opportunities to all applicants for employment regardless of an individual’s characteristics protected by applicable state, federal and local laws.</p>",
