    "description": "<h2>Overview</h2>\n<p></p>\n<p></p>\n<p>Team objectives consist of providing education and training for supervisors and direct care staff of adult care/assisted living facilities, family care homes, skilled nursing homes, community agencies such as local NAMI organizations, Senior centers, Nutrition sites, First Responders, Faith Based Organizations, Home Care Agencies, Veterans, and other invested community agencies.  </p>\n<p> </p>\n<p>#wearehiring</p>\n<p> </p>\n<h2>Responsibilities</h2>\n<p></p>\n<p></p>\n<ul>\n <li>Educate long-term care facility staff through in-service training classes.</li>\n <li>Provide consultation to Long Term Care (LTC) facilities on behavior management, crisis planning and other appropriate intervention techniques.</li>\n <li>Educate LTC facilities and families within the community how to coordinate care with psychiatrists and other mental health providers.</li>\n <li>Provide case consultations as requested by LTC facility staff.</li>\n <li>Network with LTC facilities in the Health Education and Aging Resource Team (HEART) catchment area as well as with community agencies (DSS & Ombudsman) and other relevant community stakeholders.</li>\n <li>Document all services provided.</li>\n <li>Ensure compliance with regulatory requirements, as appropriate within the scope of the program.</li>\n <li>Assist with referrals to community-based services as needed.</li>\n <li>Provide resource information to LTC staff relevant to the population.</li>\n <li>Consult/ collaborate with other health care providers as appropriate. </li>\n <li>Maintain appropriate certifications and required training necessary for licensure if applicable.</li>\n <li>Maintain equipment in operating condition.</li>\n <li>Submit reports on the required date each month for billing purposes.</li>\n <li>Record expenditures on appropriate forms and submit on the required date of each month.</li>\n <li>Travel to required agencies to provide education and training.</li>\n <li>Travel within team counties to market program services.</li>\n <li>When necessary, evening or weekend trainings will be required.</li>\n <li>Attend all required meetings pertaining to HEART.</li>\n <li>Perform other duties as required by circumstance.</li>\n</ul>\n<p> </p>\n<h2>Qualifications</h2>\n<ul>\n <li>Must be a Licensed Registered Nurse </li>\n <li>Must have at least One-year experience working with Gero-Psychiatric population </li>\n <li>Must have No Substantiated findings on the NC Personnel Registry or Health Care Program. </li>\n <li>Must have a valid NC Drivers License and maintain during duration of employment.</li>\n <li>Must have a clean SBI/DMV Report and maintain during duration of employment.</li>\n <li>Must have at least 8-years of driving experience, as this is a requirement of company's liability insurance carrier.</li>\n <li>Must be able to lift, push and pull a minimum of 25lbs.</li>\n <li>Requires repetitive lifting and carrying of approximately 15 lbs.</li>\n <li>Requires staff to sit for periods of time, walk for up to several hours at a time with breaks.</li>\n <li>Requires staff to squat and climb at times.</li>\n <li>Must sit in a moving vehicle for long periods of time.</li>\n <li>Work is primarily indoors in the home setting and out in the community, which requires exposure to various elements and road conditions</li>\n <li>Must be able to operate a vehicle in varying weather conditions</li>\n <li>Must be cognitive and utilize safety skills while walking, driving and maneuvering through various weather conditions.</li>\n</ul>\n<p> </p>\n<p><strong></strong></p>",
