Description

Overview Under the direction and supervision of the Clinical Supervisor for Crisis Services, the Clinical Case Manager works with children ages 5 – 17 in Crisis Services programs at St Joseph’s Villa. Crisis Services includes both the Crisis Receiving Center (CRC) and the Crisis Stabilization Unit (CSU). Clinical Case Manager coordinates the overall services provided to individuals with external agencies and provides targeted discharge linkages for individuals to ensure ongoing stabilization for the next appropriate level of care. All aspects of service delivery are to be conducted in alignment with the Crisis Now Model** – No Wrong Door/No Force First approach and systems-based framework with strong collaboration skills with and in support of other services offered by St. Joseph's Villa and the community (partnership with Region 4), All work is performed in accordance with the highest professional ethical standards, and in alignment with Saint Joseph’s Villa mission, values, and guiding principles. **Crisis Now Model - (all people requesting services regardless of age, etc. are to briefly be screened and linked through a warm hand-off to appropriate level of care). ESSENTIAL JOB FUNCTIONS Responsible for maintaining up to date resource lists and developing partnerships with external parties to ensure effective linkage with a warm hand-off to support crisis stabilization. Provides intensive case coordination using the High-Fidelity Wraparound model for individuals and families in coordination with Community Service Boards (CSB) and other appropriate organization including but not limited to schools, physicians, private providers, etc. to support stabilization upon discharge. Works collaboratively with the Crisis Services team to provide the highest level of treatment services to individuals and families and provide necessary direct care for individuals. May conduct referral screenings and consult with crisis services team to determine eligibility for services. May act as a liaison between tele psych psychiatrist and crisis stabilization team in order to ensure continuity of care. Works with treatment team to develop action plans and crisis prevention plans based on the family’s prioritized needs. Works collaboratively with crisis services team based on the Individual’s and family’s prioritized needs to identify effective treatment, develop safety plans, and discharge planning for continuous stabilization upon discharge. Researches community-based organizations and governmental agencies to determine what resources are available to support the individual’s needs. Actively links individuals and their families to these identified resources. Maintains case management documentation. Compiles information to prepare discharge summary reports of individuals. Maintains contact with designated family members or guardians and school systems. Coordinates community support and intra-agency support as needed for each individual to ensure effective linkage with a warm hand-off to support crises stabilization. Models high level of professionalism with appropriate communication with individuals and staff, behavior, and dress. Attends and participates in All-Staff Meetings, weekly Treatment Team Meetings, training, and other events/meetings that support the operation of Crisis Services Evaluates work processes and methods and submits recommendations for continuous quality improvement. Attends scheduled training programs for professional development that includes, at a minimum, trainings required by St. Joseph’s Villa and by regulatory and accrediting bodies. ADDITIONAL DUTIES AS ASSIGNED BY SUPERVISOR May participate in individual intake, admission, and discharge process May coordinate and participate in tele-health psychiatry services May administer medication. May provide direct individual care as needed. When necessary, may be deployed to other services at St Joseph’s Villa to provide appropriate needed services. Performs other duties as assigned. QUALIFICATIONS BS/BA degree in Psychology, Human Services, or related field. A master’s degree is preferred. Two years of related experience. One year of clinical experience with children (QMHP preferred). Proficient with Windows, Outlook, MS Office including Word, Excel and PowerPoint. Experience with proprietary software preferred. Valid Virginia driver’s license with excellent driving record. KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of the philosophy and practices of therapeutic programs for the emotionally, behaviorally, and cognitively impaired individual. Knowledge of appropriate methods of de-escalation and ability to implement those methods appropriately. Knowledge of Medicaid Billing procedures. Able to work effectively with parents or guardians and placing agencies. Excellent verbal communication skills. Proficient in basic mathematics and elementary statistical techniques. Able to organize case load to maximize time utilization with no diminution in quality of service. Able to appropriately use proprietary software. Able to work effectively in a collaborative, shared leadership environment. Able to be flexible and appropriately handle stressful situations. Responsibilities Qualifications BS/BA degree in Psychology, Human Services, or related field. Two years related experience. One year of clinical experience with children (QMHP-C preferred).
