We also provide attractive working conditions and opportunities for career growth through professional development.</p>\n<p>Discover why Cooper University Health Care is the employer of choice in South Jersey.</p>\n<h2>Short Description</h2>\n<p>Implement, and evaluate individualized care plans for each patient under the direction of the Nurse Practitioner and attending physicians.</p>\n<p>Educate patients on appropriate self management behaviors and skills for disease management.  </p>\n<p>Maximize patient health outcomes and the patient experience by supporting a coordinated plan of care during and between clinic visits.</p>\n<p>Coordinate various pre-visit and post-visit planning and scheduling to form relationships that keep patients engaged in consistent care. </p>\n<p>Assist in organizing and conducting group visits.</p>\n<p>Assist in optimizing training materials, patient communications, and other documents for program managers.</p>\n<p>Participate in team case conferences to address progress and barriers in patient care.Maintain licensure.</p>\n<p>Other duties and responsibilities as directed.</p>\n<h2>Experience Required</h2>\n<p>1‐3 years experience providing clinical servicesin an outpatient setting preferred but will consider new graduate.</p>\n<h2>Education Requirements</h2>\n<p>LPN degree from Accredited Nursing Program required.</p>\n<h2>License/Certification Requirements</h2>\n<p>Current NJ-LPN.</p>\n<p>BLS required. </p>\n<h2>Special Requirements</h2>\n<p>Bilingual English/Spanish;Excellent organizational and interpersonal skills; strong oral/written communication skills is a must;Demonstrates ability to work well with people of various backgrounds, ethnicities, and life experiences;Ability to work collaboratively in a team and manage multiple priorities, utilize effective time management skills, and exercise sound administrative and clinical judgment;Demonstrates ability to act independently;Ability to work in multiple computer systems.</p>\n<h2>Salary Min ($)</h2>USD $25.00\n<h2>Salary Max ($)</h2>USD $42.50",
