Description
Job Overview
This is a union position.
STOP Program Area Six provides comprehensive, evidence-based programming and services for California Department of Corrections and Rehabilitation inmates and parolees to utilize during their transition back into society. Area Six includes San Diego, Orange, and Imperial counties. Priority program placements are given to those participants who are within their first year of release and who have been assessed for moderate to high risk of re offending.
Community Service Representatives (CSRs) provide a comprehensive array of placement, reentry, and case management services to the California Department of Corrections and Rehabilitation (CDCR) population from within the in-prison Substance Abuse Treatment (SAT) programs, Community Transition Program (CTP), and other General Population (GP) inmates scheduled for release from CDCR institutions. The CSR screens and manages the participant from the intake process for placement into Community Based Providers (CBP), through treatment, and into the final stages towards an independent, crime-free lifestyle. The position also requires service provision to a caseload of CDCR participants, ongoing data entry of placement notes, filing, coordination of CBP placement, and transportation planning with institutional staff and other duties as assigned by STOP supervisors.
Key Responsibilities
Conduct outreach presentations within CDCR institutions and interaction and coordinating documentation with CDCR personnel. Maintains accurate and comprehensive documentation on each prospective STOP client. Attends required trainings and meetings. Assist in training of new hires. Coordinates with available internal and external resources to develop/maintain reentry plans and transition plans. Maintains accurate records by data entering documentation into various electronic systems for all prospective participants in accordance with established guidelines. Assess, schedule and coordinate placements of referrals into appropriate community placements in conjunction with the STOP Placement Coordinators and Transportation Coordinators, and their supervisors. Coordinates transportation with STOP Transportations Coordinator(s). Match inmates to the appropriate level of care within a network of sub-contracted Community Based Providers. Conduct regular communication with Parole Agents. Work collaboratively with CDCR staff while preserving the safety and security of CDCR institutions. Other Responsibilities: Due to security concerns at institutions and/or modification in workload via CDCR changes in the contract, may be assigned “other than CSR position duties” by STOP supervisors as needed.
Education and Knowledge, Skills and Abilities
High School Diploma or equivalent. A valid California driver’s license. Minimum of (3) years experience working with a criminal justice population. First Aid Certified and CPR Certified Must pass HealthRIGHT360 computer test. Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data. Professionalism, punctuality, flexibility and reliability are imperative. Excellent verbal, written, and interpersonal skills. Integrity to handle sensitive information in a confidential manner. Excellent organization skills and ability to multitask and juggle multiple priorities. Outstanding ability to follow-through with tasks. Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility. Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations. Able to work within a frequently changing project scope while maintaining overall direction and structured priorities. Must undergo a background investigation and criminal records check to obtain clearance. Must be able to satisfy requirements for security clearances at CDCR Institutions. Must not be on active parole or probation. If previously on parole or probation, must be off for a minimum of 3 years. Culturally competent and able to work with a diverse population. Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications. Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency. We will consider for employment qualified applicants with arrest and conviction records.
