No late nights, no weekends, and 11 paid holidays.</span></p>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">General Function:</span></p>\n<p><span>Responsible for providing quality medical care, specific to specialty, to NHA patients.</span></p>\n<p><span style=\"font-weight: bold\">Position Overview:</span></p>\n<p>Within the Patient-Centered Medical Home (PCMH) Care Team, Certified Nurse Practitioners collaborate closely with patients and other healthcare professionals to coordinate care and support shared goals. These goals include improving patient access, delivering high-quality care, and empowering patients to effectively manage their own health.</p>\n<p><span style=\"font-weight: bold\">Key Responsibilities:</span></p>\n<ul>\n<li>Actively participate in pre-visit team huddles to review patient schedules, previous notes, lab updates, and needed preventive services.</li>\n<li>Attend monthly team meetings focused on high-risk patients, staffing, role clarification, team building, and quality improvement.</li>\n<li>Follow the PCMH Patient Flow Chart to streamline care, reduce delays, and improve outcomes and cost-efficiency.</li>\n<li>Provide medical services within the nurse practitioner’s scope of training and as directed by the Medical Director.</li>\n<li>Maintain accurate patient records and comply with NHA regulations, medical laws, ethics, and administrative reporting standards.</li>\n<li>Communicate with the Chief Medical Officer regarding malpractice insurance matters.</li>\n<li>Participate in required medical staff, case management, and other assigned meetings, including Quality Assurance Committee duties when applicable.</li>\n<li>Understand and follow the Center’s Fire and Disaster Plan.</li>\n<li>Take part in peer chart reviews as assigned.</li>\n<li>Perform additional duties as required and maintain regular attendance.</li>\n</ul>\n<p><span style=\"font-weight: bold\">Qualifications:</span></p>\n<ul>\n<li><span style=\"font-weight: bold\">Active Ohio License:</span> Must hold a current and valid license in the State of Ohio.</li>\n<li><span style=\"font-weight: bold\">Certificate of Authority:</span> Required in the applicable field of practice.</li>\n<li><span style=\"font-weight: bold\">CPR Certification:</span> Current CPR certification is mandatory.</li>\n<li><span style=\"font-weight: bold\">Language Skills:</span> Bilingual proficiency in English and Spanish is preferred.</li>\n<li><span style=\"font-weight: bold\">Experience:</span> Prior experience working with medically indigent populations is highly desirable.</li>\n<li><span style=\"font-weight: bold\">Professional Competence:</span> Must demonstrate compassion, clinical competence, strong work ethic, and the ability to work effectively as part of a team.</li>\n<li><span style=\"font-weight: bold\">Cultural Sensitivity:</span> Must be respectful and sensitive to cultural, religious, and ethnic diversity.</li>\n<li><span style=\"font-weight: bold\">Communication &amp; Organizational Skills:</span> Exceptional written, verbal, and listening skills, along with strong organizational abilities, are essential.</li>\n<li><span style=\"font-weight: bold\">Emotional Intelligence:</span> Must exhibit personal maturity and the ability to perform well under challenging clinical circumstances.</li>\n<li><span style=\"font-weight: bold\">Professional Appearance:</span> A neat, professional, and appropriate appearance is required.</li>\n</ul>\n<p><span style=\"font-weight: bold\">Who We Are:</span></p>\n<p>Neighborhood Health Association (NHA) is Northwest Ohio’s largest community health center system.
