This role combines infusion therapy and patient education, providing both in-home care and valuable support to patients undergoing treatments such as TPN, IVIG, and IV biologics.</span><br></p>\n<p><span>The RN will be responsible for providing telephone and in-person advice and education to patients. This role requires offering interventions, counseling, and support services that require significant nursing knowledge, judgment, and skills in psychological, biological, physical, and social sciences. Under the supervision of the Director of Nursing, the RN will make decisions based on their education, nursing experience, and clinical guidelines, ensuring optimal compliance and persistence in home infusion programs.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Schedule Details &amp; Locations – Indiana</span></p>\n<ul>\n<li><span>Per Diem, Flexible Hours</span></li>\n<li><span>Independent Contractor</span></li>\n<li>We are seeking Registered Nurses<span> </span><span style=\"font-size: 12pt\">for the following locations:</span><br>\n<ul>\n<li>Hamilton, IN</li>\n<li>Logansport, IN<br></li>\n<li>Valparaiso, IN</li>\n</ul>\n</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Duties and Responsibilities</span></p>\n<ul>\n<li><span>Conduct in-home infusion therapy visits with patients according to the plan of care.</span></li>\n<li><span>Prepare clinical/nursing notes and document patient interventions in both clinical and non-clinical records consistently.</span></li>\n<li><span>Act as a clinical resource, working collaboratively with other pharmacy departments.</span></li>\n<li><span>Maintain knowledge of drug side effects and interactions.</span></li>\n<li><span>Offer telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures.</span></li>\n<li><span>Ensure patients understand their therapy plans, medications, and the importance of adherence.</span></li>\n<li><span>During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process.</span></li>\n<li><span>Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.</span></li>\n<li><span>Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs.</span></li>\n<li><span>Manage patient and family expectations regarding services, outcomes, and discharge goals.</span></li>\n<li><span>Establish primary and secondary diagnoses based on patient assessment and home health care focus.</span></li>\n<li><span>Develop care plans with skilled interventions, medical supplies, and necessary services to meet goals.</span></li>\n<li><span>Regularly evaluate patient progress, in collaboration with team members, and revise care plans as needed.</span></li>\n<li><span>Perform other duties as assigned.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Qualifications</span></p>\n<ul>\n<li><span>Graduate of an accredited nursing school with a current RN state license.</span></li>\n<li><span>2+ years of experience in home infusion and infusion centers.</span></li>\n<li><span>2+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified Registered Nurse Infusion) is preferred.</span></li>\n<li><span>Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred</span></li>\n<li><span>Effective interpersonal, time management, and organizational skills.</span></li>\n<li><span>Strong written and verbal communication skills.</span></li>\n<li><span>Organizational ability to maintain consistently accurate records.</span></li>\n<li><span>Ability to direct information to the appropriate healthcare provider.</span></li>\n<li><span>Proficient in computer skills, including word processing, and efficient use of the internet and email.</span></li>\n<li><span>Current CPR and BLS (Red Cross and AHA) certification, valid driver’s license, reliable transportation, and automobile insurance.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">AmeriPharma’s Mission Statement </span></p>\n<p><span>Our goal is to achieve superior clinical and economic outcomes while maintaining the utmost compassion and care for our patients. May frequently move about inside and outside the work area (patient's home) to access medical equipment. Must be physically able to operate a computer and other office productivity machinery (i.e., a laptop, phone, etc.).  Must be able to consistently communicate with patients and team including via telephone and texting. Must be able to lift up to 20 pounds at times.
