    "description": "<p><span style=\"font-weight: bold\">Job Highlights</span></p>\n<p><br></p>\n<p><span style=\"font-size: 16px\">The Physical Therapist is a qualified professional person who directs, supervises, evaluates and provides Physical Therapy services to clients in the home as prescribed by the Attending Provider.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Qualifications</span></p>\n<ul></ul>\n<p><span style=\"font-size: 15px\">Graduate of a Physical Therapy school approved by:</span></p>\n<ul>\n<li><span style=\"font-size: 15px\"> The American Physical Therapy Association; or</span></li>\n<li><span style=\"font-size: 15px\"> The Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association; or</span></li>\n<li><span style=\"font-size: 15px\"> The Council on Medical Education of Hospitals for the American Medical Association</span></li>\n<li><span style=\"font-size: 15px\"> Licensed or registered by the applicable state</span></li>\n<li><span style=\"font-size: 15px\"> Preference of at least one (2) year of experience</span></li>\n<li><span style=\"font-size: 15px\"> Excellent interpersonal communication skills.</span></li>\n<li><span style=\"font-size: 15px\"> Proficient in English and Spanish.</span></li>\n<li><span style=\"font-size: 15px\"> Compassionate and attentive to detail.</span></li>\n<li><span style=\"font-size: 15px\"> Possess up-to-date knowledge of treatment practices.</span></li>\n<li><span style=\"font-size: 15px\"> Ability to solve problems independently.</span><br></li>\n<li><span style=\"font-size: 15px\"> Ability to write reports and correspondence at a professional level.</span></li>\n<li><span style=\"font-size: 15px\"> Ability to use a computer and software proficiently within Wellsky (formerly Kinnsar) EMR.</span></li>\n</ul>\n<p><span style=\"font-size: 16px\"> </span></p>\n<p><span style=\"font-weight: bold\">Responsibilities</span></p>\n<ul>\n<li><br><span style=\"font-size: 15px\">Assists the provider in evaluating clients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests</span></li>\n<li><span style=\"font-size: 15px\"> Treats clients to relieve pain and/or develop or restore function to maintain maximum performance</span><br></li>\n<li><span style=\"font-size: 15px\"> Records and reports to the provider the client's reaction to treatment and/or any changes in the client's condition</span></li>\n<li><span style=\"font-size: 15px\"> Instructs clients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, wheelchairs, etc.)</span></li>\n<li><span style=\"font-size: 15px\"> Instructs the family in the client's total physical therapy program</span></li>\n<li><span style=\"font-size: 15px\"> Attends Physical Therapy staff meetings and in-service presentations</span></li>\n<li><span style=\"font-size: 15px\"> Attends case conferences and in-service programs as indicated</span></li>\n<li><span style=\"font-size: 15px\"> Supervises the Physical Therapy Assistant, as indicated</span></li>\n<li><span style=\"font-size: 15px\"> Participates in staff and Agency development activities</span></li>\n<li><span style=\"font-size: 15px\"> Prepares an assessment of the client's condition, OASIS information, if indicated, and periodic evaluations and summaries, develops a plan of care and problem list, and writes clinical/progress notes on the date of the visit and incorporates same in the client's record no less than weekly</span></li>\n<li><span style=\"font-size: 15px\"> Participates in the development and periodic revision of the provider's Plan of Care</span></li>\n<li><span style=\"font-size: 15px\"> Communicates effectively with all those providing care</span></li>\n<li><span style=\"font-size: 15px\"> Confirms, on a weekly basis, the scheduling of visits with the DON to coordinate necessary visits with other personnel</span></li>\n<li><span style=\"font-size: 15px\"> Notifies Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings which will affect agreed services with the Agency</span></li>\n<li><span style=\"font-size: 15px\"> Participates with staff, client, and provider in discharge planning activities and completes the Physical Therapy Discharge Summary within the designated framework of Agency policies</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Benefits</span></p>\n<ul>\n<li><span style=\"font-size: 15px\">Pay: Varies by visit</span></li>\n<li><span style=\"font-size: 15px\">Location: In-person</span></li>\n<li><span style=\"font-size: 15px\">Job Type: Full time</span><br></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Special Requirements</span></p>\n<ul>\n<li><span>Must have a car with required insurance coverage and a state driver's license or reliable transportation</span><br></li>\n<li><span>Carries malpractice insurance</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Functional Abilities</span></p>\n<ul>\n<li><span style=\"font-size: 15px\">Able to communicate verbally and in writing to the extent required by the position</span></li>\n<li><span style=\"font-size: 15px\">Able to physically perform the duties required by the position within Miami-Dade and Monroe County</span></li>\n<li><br><span style=\"font-size: 16px\">Able to travel to prospective clients' residence</span><br></li>\n</ul>",
