Description
Zero Suicide is a framework to improve suicide care. Within the Zero Suicide framework, we believe that suicide deaths of individuals in a healthcare system are preventable, and we strive to prevent suicide attempts and suicide deaths. At Prairie View, we have implemented a Zero Suicide pathway for individuals at risk for suicide. This pathway is designed to increase awareness, care, and support for individuals at risk for suicide.
Job Summary:
As the Zero Suicide coordinator, you will have direct contact with patients who are at risk for suicide to provide outreach, resources, and support with a compassionate and patient-centered approach. Working collaboratively with patients, families, internal providers, and external providers to coordinate care will be essential. In addition to supporting patients who are at risk of suicide, the coordinator is responsible for managing administrative tasks related to Zero Suicide, including enrolling and disenrolling patients from the Zero Suicide pathway, assisting with Zero Suicide data tabulation (e.g., managing enrollment reports), and reviewing adherence to Zero Suicide procedures. The Zero Suicide coordinator must be able to manage both patient-centered and administrative responsibilities.
POSITION RESPONSIBILITIES:
Learn and effectively understand the key components of Zero Suicide
Conduct outreach and regular check-ins with individuals enrolled on the Zero Suicide pathway
Manage enrollment and disenrollment of individuals on the Zero Suicide pathway
Manage collection of data for grant reporting and other organizational purposes
Be an active participant in the Zero Suicide Team and attend regularly scheduled meetings
Provide communication to staff related to Zero Suicide program and patients as needed
Assisting and coordinating with organizational team members regarding care of individuals enrolled on the Zero Suicide pathway
Reviewing documentation and adherence to Zero Suicide procedures
Participate in professional workshops, seminars, and conferences to build skills and expand professional knowledge regarding Zero Suicide
Participate in continuing education activities and required training (such as Infection Control, etc.) as requested, to enhance and improve job performance
Utilize Essential Learning site to enhance knowledge in the areas of mental health and treatment modalities
Maintains confidentiality in all aspects of position
Uses problem situations as learning opportunities
Brings energy and positive attitude to the work place
Other duties as assigned
Qualifications
Minimum Education : Bachelor’s degree in related field of behavioral sciences or equivalently qualified by work experience or a combination of work experience in the human services field and education
Minimum Experience : Preferred experience working directly with behavioral health populations
Must be at least 21 years of age
Demonstrate effective interpersonal skills; ability to work with a wide variety of individuals with behavioral health needs served by a CCBHC and ability to react effectively in a wide variety of human service situations.
Must have good listening skills, verbal skills, and written communication skills
Must have good organizational skills to be able to plan and prioritize contacts with patients
Must be able to observe patient demeanor and non-verbal communication for assessment purposes
Must be able and willing to work flexible hours and adjust work schedule to meet patient and organization needs
Must be able to communicate effectively with referral sources, external care providers, and mental health center team members (i.e. case managers, therapists, physicians, community support workers)
Must have the ability to provide empathy, compassion, and hope for individuals at risk for suicide and believe they can build lives worth living
Must have the ability to work independently and as a productive member of a team to develop a therapeutic rapport with diverse individuals.
Must be proficient at basic keyboarding/word-processing skills
Preferred experience with Excel
Must be able to pass Kansas Bureau of Investigation Screen, Central Registry Screen, and DCF (Kansas Dept. for Children & Families, formerly SRS) background check
