The provider will coordinate with other behavior health and primary medical providers and support care management within the Patient Centered Medical Home (PCMH) integrated model.</p><p><strong>Position Details</strong></p><ul style=\"margin-left: 8px \"><li><strong>Location:</strong> Rotating (Fayetteville, Siloam Springs, and Rogers)</li><li><strong>Employment Status</strong>: Full-Time</li><li><strong>Department:&nbsp;</strong>Behavioral Health</li><li><strong>Reports to:&nbsp;</strong>Behavioral Health Clinical Manager</li><li><strong>Classification:</strong> Exempt (Salary)</li></ul><p><strong>Essential job functions</strong></p><ul style=\"list-style-type: disc;\"><li>Provide clinical services as defined at the Community Clinic or through sites designated for the provision of health care services by the Clinic</li><li>Use current evidence-based practice, research and clinical knowledge to improve patient outcomes and reduce complications.</li><li>Formulate behavioral health treatment plan and manages acute and chronic mental health needs within Primary Care Mental Health model of care.&nbsp;</li><li>Provide treatment to clients with drug/opioid addiction.&nbsp;</li><li>Utilize all Clinic resources to improve patient outcomes, including Behavioral Health Consultants, Patient Educators, etc.</li><li>Provide assessment and medication consultation for primary care and behavioral health providers at for patients with complex behavioral health, developmental, or substance disorders.&nbsp;</li><li>Order laboratory and other diagnostic tests and evaluates data obtained.</li><li>Practice within the scope of competency as an Advanced Practice Registered Nurse (APRN) in the State of Arkansas and meets Board requirements for licensure.</li><li>Cooperate with administrative duties such as: credentialing, clinic certifications, medical staff development, and representation within the community.</li><li>Enter clinical notes in the patient&rsquo;s medical record accurately and in a timely manner</li><li>Participate in Quality Improvement practices in the clinic, e.g. chart and peer reviews.</li><li>Meet or exceed minimum productivity expectations.</li><li>Adhere to the core values of Community Clinic and reinforce with staff and the community.</li><li>Maintain continuing education at board-required levels.</li><li>Adheres to applicable regulatory guidelines and laws, including but not limited to HIPAA/HITECH, CLIA, NCQA PCMH HRSA and OSHA.&nbsp;</li><li>Other duties related to the Behavioral Health program as assigned.</li></ul><p><strong>Community Clinic Essential Competencies</strong></p><ul type=\"disc\"><li>Service: Treats every patient and coworker with empathy, respect, and a genuine desire to help.</li><li>Ownership: Takes responsibility for their actions, follows through on commitments, and upholds high standards even when no one is watching.</li><li>Adaptability: Remains flexible, open to change, and willing to learn in order to meet the evolving needs of our patients, teams, and organization.</li><li>Collaboration: Works together with openness and respect, shares information freely, and supports co-workers to provide seamless, high-quality care.</li><li>Problem Solving: Approaches challenges with curiosity, finds practical solutions, and takes initiative to improve processes and patient experiences.</li></ul><p><strong>Role Specific Essential Skills/Competencies</strong></p><ul style=\"list-style-type: disc;\"><li>Psychiatric Assessment and Diagnosis: Evaluates and diagnoses mental health, substance use, and developmental disorders.</li><li>Medication Management: Prescribes and monitors psychiatric medications safely and effectively.</li><li>Integrated Care: Collaborates with primary care and behavioral health teams to coordinate care.</li><li>Evidence-Based Practice: Applies current research and best practices</li><li>Documentation: Maintain accurate records and follows regulatory and licensure requirements.</li><li>Substance Use Treatment: Able to Assess and treat patients with substance use disorders.</li><li>Autonomy/Collaboration Balance: Able to perform autonomously and exercise independent judgment.
