Quality Moments is looking for an individual whose desire to support the growth and well-being of youth aligns with our value of personal connection, keeping commitments, and putting people first in our lives.</p>\n<p><span style=\"font-weight: bold\">Qualifications and Responsibilities of Senior Care Case Manager</span></p>\n<ul>\n<li>3+ years experience int the field of social services, human services, or a related field.</li>\n<li>Desire to improve quality of life for vulnerable populations.</li>\n<li>High level of commitment, dependability, and interpersonal skills.</li>\n<li>Prior experience in social work, counseling, human services, or a related field is preferred.</li>\n<li>Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.</li>\n<li>Previous EHR system use, and professional documentation experience are helpful.</li>\n<li>Computer skills are required for documentation and system use.</li>\n<li>Associate's degree preferred.</li>\n</ul>",
