Summary: The Crisis Discharge Planner will be responsibility to discharge and provide follow-up resources for all clients admitted to the Unit (CSU/Respite/Detox). Education Requirement: Bachelor’s degree in Psychology, Human Services or related discipline from an accredited University. Experience Requirement: Experience working in a mental health or crisis setting. Mental health or crisis experience preferred. Licensure/Certification Requirement: None Key Job Responsibilities: Schedule all aftercare appointments for clients discharging from the Unit. Coordinate referrals to treatment, housing, or supportive services for clients discharging from the Unit. This includes but is not limited to treatment facilities, homeless shelters, group homes, work programs, supportive living facilities, etc. Arrange transportation as needed for clients discharging from the Unit. Bill appropriate service for each visit completed with client (THL, BHSN CM, etc.) Update census spreadsheet each morning to prepare for discharges for the day and week. These should be printed and passed out to the Unit Coordinator, Unit Clerk, Providers, Therapist, and MHTs as needed. Complete daily discharge list for all clients who discharged previous day and email for completion of follow-up calls. Complete Utilization Review on all TennCare clients after they discharge from the Unit. Facilitate groups consistent with credentialing and degree (IMR, therapy, etc.) for clients on the Unit. Ensure each client discharging completes and returns a client satisfaction survey (that is later given to the Managers). Participate in all required trainings. Read work email every business day. Abide by the policies and procedures as outlined in the Policy and Procedure Manual Knowledge/Skills/Abilities: This person must be able to work effectively in a crisis service delivery setting, exemplify problem solving skills, formulate appropriate crisis intervention response, demonstrate self-motivation, and possess excellent communication skills and the ability to work well with others. This person must contribute to a professional, healing, and restorative environment. PERSONAL RELATIONSHIPS In order to appropriately accomplish his/her duties, the person occupying this position will be required to maintain effective relationships with local hospitals and other referring partners, Crisis frontline staff, Crisis Managers, Alliance Directors, and the clients of Alliance Healthcare Services Supervision Provided by this Job: None Physical Demands/Conditions: Prolonged standing and walking; lifting, turning patients and equipment. Heavy Work - Exerting up to 150 pounds of force occasionally, and/or up to 75 pounds of force frequently, and/or up to 30 pounds of force constantly to move objects. (May be further defined as "with or without assistance.") . 8am-4pm (2 8-hour shifts)
