The Social Worker provides services as a member of a multidisciplinary team. The incumbent will work collaboratively with community groups, agencies, and other professionals within the community. Human Resources Exemption: No Education Bachelor of Social Work (BSW) Knowledge and Abilities Knowledge in individual assessment and counselling skills Knowledge in local human service organizations, resources, referral procedures Knowledge in health promotion and prevention strategies Committed to delivering on patient and family centred care Expert knowledge and advanced skill in a variety of treatment modalities Strong interpersonal skills Systems Theory/Family Systems Theory, strength-based model, crisis intervention Ability to work cooperatively in an integrated interdisciplinary team Committment to self evaluation, professional development and ongoing self care Understanding social determinants of health for program development & delivery Ability to complete case management assessments, follow-up and treatment Ability to work with a variety of individuals presenting with complex issues Demonstrates strong communication and organizational skills Other Information Valid Class 5 driver's license One year experience in medical social work preferred Registered/eligible with the Saskatchewan Association of Social Workers (SASW) Access to a reliable vehicle Organization Applicants must upload all relevant documents (ex: certificates, diplomas, proof of enrollment, licenses and/or memberships) relating to the qualifications of the position.
