Who You Are:
Providing counseling and case management services related to substance abuse, legal, mental health, medical and vocational/educational issues are the reasons you are pursuing a career within social services. You are a collaborator and forward thinker, who has maintaining compliance as their first thought when handling client cases. Your belief that a healthy lifestyle is a drug-free one and strive to be a role model to the clients that you serve. You are a self-motivated, resilient person who thrives in a clinical environment. You enjoy working within a team that is diverse, as well with communities that are vulnerable in nature, in order to deliver culturally competent services to a varying client population in an urban setting. You believe that treatment services should be based on individualized treatment plans that pertain to chemical abuse/dependency and that other affected life areas including discharge planning needs need to be assessed on the one-on-one client level. You are a team player, who is highly organized and has time management skills. You are well versed in technology and have excellent communication skills. Travelling locally and having a flexible schedule, which may include working evenings, weekends or holidays, does not bother you. What You’ll Need:
CASAC or CASAC-T required. (A Master’s Degree in a counseling related field preferred). CCAR Recovery Coach certification required. Candidates who do not have this certification must obtain the standard within 6-month probationary period. Proficiency with computer operation (Microsoft Word, Excel, Electronic Health Records, and Outlook programs). Must be willing to spend 35% of work time in the community delivering services in client’s homes, conducting outreach, and making school visits. What You’ll Be Doing:
Administer psychosocial assessment tools (i.e. GAIN, Comprehensive Evaluation) at various points in time within regulatory required time frames. Provide individual counseling and case management services onsite and in the home or community, addressing all medical, psychiatric, housing, vocational, and other life area concerns. In collaboration with each client, formulates an individualized treatment plan, monitors achievement of objectives, reviews and updates the treatment plan in accordance with regulatory requirements and as clinically indicated. Maintain up-to-date client clinical records including at minimum: treatment plans, group counseling notes and progress notes in accordance with regulatory guidelines and program standards. Conduct education/prevention groups to address topics which address chemical dependency and related issues. Provide crisis intervention services. Responsible for ongoing client discharge planning. Provide other therapeutic services through active participation, observation and evaluation of each client’s assigned goal related and recovery focused tasks including continuity of care coordination. Facilitate evidenced based groups and services per level of curriculum or individual training. Operate as a liaison with all referral agencies providing regular verbal and written progress reports. Establish community and agency affiliations which address the client’s need and strengthen the delivery of services. Provide education and direct services to family members, as appropriate, in regards to client’s substance abuse and related behavioral issues. Conduct collection of samples for urine toxicology and other forms of testing. Attend regularly scheduled staff meetings and case conferences. Attend all required in-service training seminars. Participate in quality improvement activities. Other relevant duties as required.
