administers&nbsp;prescribed&nbsp;medications&nbsp;via&nbsp;all&nbsp;potential&nbsp;routes&nbsp;of&nbsp;administration</li><li>Assists with transferring individuals in and out of vehicles and into the facility at arrival and departure.&nbsp;</li><li>Performs emergency care, including but not limited to calling 911 for Life Threatening Emergencies and rendering lifesaving interventions such as CPR &amp; First Aid until EMT&rsquo;s arrive, in compliance with Danielle&rsquo;s Law.&nbsp;</li><li>Maintains and documents using the Medication Administration Record (MAR)/ Electronic MAR (E-MAR) to ensure that each individual receives prescribed routine and PRN medications and treatments on time.</li><li>Reports &amp; documents changes in the individual&rsquo;s medical condition to supervisors,&nbsp;guardians,&nbsp;residential&nbsp;managers,&nbsp;and&nbsp;the&nbsp;supporting team in accordance with&nbsp;policy and regulatory requirements.</li><li>Accurately adheres to and maintains current physician&rsquo;s orders, annual medical documentation, necessary&nbsp;program and other required documents reflecting changes in the individual&rsquo;s medical plan in compliance with Individual&nbsp;Service&nbsp;Plan (ISP)&nbsp;&amp; Interdisciplinary (IDT) Plans.</li><li>Adheres to Physician&rsquo;s Orders for Life Sustaining Treatment (POLST) and supports individual&rsquo;s&nbsp;receiving end&nbsp;of&nbsp;life/hospice&nbsp;services.</li><li>Works as a team with other personnel to &nbsp;promote a pleasant, therapeutic, and goal-oriented experience.</li><li>Participates in IDT, ISP, Consultations, agency meetings &amp; training sessions.</li><li>Assists and monitors direct support professionals in caring for the individual&rsquo;s comfort and safety needs as needed.</li><li>Assists other staff with individualized, professional therapeutic plans and prescriptions, such as therapeutic lifting, positioning, and transfer requirements, as well as while maintaining safety precautions, such as aspiration precautions.&nbsp;</li><li>Maintains&nbsp;a&nbsp;professional&nbsp;rapport&nbsp;with&nbsp;the&nbsp;individual&rsquo;s&nbsp;supportive&nbsp;team,&nbsp;including but&nbsp;not&nbsp;limited&nbsp;to guardians, parents, residential managers, physician&rsquo;s, therapists, nurses and other professional caregivers to&nbsp;support&nbsp;the&nbsp;individual&rsquo;s&nbsp;health,&nbsp;safety,&nbsp;medical&nbsp;and&nbsp;dietary&nbsp;needs.</li><li>Maintain confidentiality&nbsp;</li><li>Comply with the DDD Program Manual, The Arc Gloucester Policies and Procedures Manual and applicable state, local and federal regulations, including any COVID-19 vaccination mandates &amp;
