Description
Community Health Action of Staten Island (CHASI ) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community.
POSITION SUMMARY:
The Vice President for Programs is a member of Community Health Action of Staten Island (CHASI)’s leadership team responsible for planning, implementation, service monitoring, and ongoing development of the Health Care Coordination department and Criminal Justice Initiatives.
The VP is responsible for monitoring the health and social service needs and service system affecting the CHASI clients living with chronic health or behavioral health conditions, HIV or justice involvement, refining program operations to address emerging trends. The VP provides strategic direction, promotes best practices, and ensures efficient operation of service delivery. The VP supervises department managers and other relevant department staff.
In addition, the VP is responsible for the program-wide data department which is responsible for timely and accurate data entry and reporting compliance. Working with the senior leaders, the VP is also responsible for promoting, monitoring and assessing the implementation of the quality management program.
DUTIES & RESPONSIBILITIES:
Strategic Program Development
Identify program needs and funding opportunities to address those needs. Participate in the development of funding proposals. Identify collaborations to promote client recruitment into department services, establish linkages and referral pathways. Design program workflows and systems to foster integration of internal service delivery that addresses the holistic needs of CHASI’s clients. Participate in County and City-wide coalitions and workgroups to address the needs of clients living with chronic health conditions, including HIV and those who are involved in the criminal justice system. Keep abreast of issues in the fields of Medicaid reimbursed and grant funded care management, HIV, behavioral health and criminal justice.
Program Oversight
Provide supervision, coaching and direction to department managers. Monitor progress against deliverables, as well as department-wide goals. Oversee department training and staff development. Initiate new projects as needed to meet the needs of the target populations. Oversee the development and update of policy and procedure manuals for the department. Prepare program reports as needed for executive leadership and the Board of Directors. Monitor completion of funder reports by program management.
Quality Management
Monitor compliance with the quality management plan developed for department. Provide coaching, training and ongoing guidance to ensure continuous quality improvement objectives are met. Work with program leadership to use program data and quality assurance reports to develop rapid-cycle projects to improve processes and outcomes. Monitor the implementation of the departmental quality assurance process and system to ensure data integrity and audit readiness. Using findings and gaps identified, develop a training plan for staff and management as needed.
Accountability
Monitor staff time and attendance. Develop a system to routinely evaluate performance against targets, identifying areas for improvement and documenting best practices that can be shared internally and with funders and other stakeholders Identify issues that need to managed collaboratively with Human Resources
Staff Development
Identify training needs of staff and management and develop a training plan each year to enhance staff competencies, knowledge and skills. Service Continuum Development
Working agency leadership, identify areas for interdepartmental collaboration. Identify and develop memoranda of understand and linkage agreements with other organizations that address CHASI’s myriad program needs. Work with Administration and program leadership on new project starts to ensure the timely execution of new services. Participate in and represent CHASI at local, state, and national conferences, task forces, coalitions, etc. focusing on case management, harm reduction, drug treatment, behavioral health, domestic violence, and other service arenas. Keep current with changes and trends in programmatic areas. The VP is responsible for the following service areas: Grant funded Care Coordination contracts Alternatives to Incarceration program New programming as determined in collaboration with the CPO.
