CarePartners is proud of its commitment to quality, relentlessly seeking to improve the patient and caregiver experience, promoting a healthy and resilient workplace culture, and contributing to a sustainable healthcare system in Ontario.</p>\n<h2>Overview</h2>\n<p>CarePartners has embarked on a new program in partnership with the Ontario Health at Home (OH@Home) Central East (<strong>Durham Region</strong>) and we are looking for Community Support Workers (CSWs) to work within the <strong>High Intensity Supports at Home (HISH) </strong>program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in <strong>Wrap-Around Care</strong> so that <em>all</em> team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, etc.) are working cohesively to meet the needs of the patient. These PSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).</p>\n<p> </p>\n<p><strong>What Makes this Position so Exciting?</strong></p>\n<p> </p>\n<p>In this exciting staffing model, Guaranteed HISH CSWs will have <strong>guaranteed &</strong> <strong>consistent hours of work</strong>, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers. </p>\n<p> </p>\n<p>The hours of work will be <strong>7, 12-hour shifts within a 2-week rotation inclusive of weekends</strong>:</p>\n<p><strong>Two-week Rotation</strong></p>\n<p><strong>Mon/Tues/Sat/Sun</strong></p>\n<p><strong>0800 - 2000 hrs</strong></p>\n<p><strong>Wed/Thurs/Fri</strong></p>\n<p><strong>0800 - 2000 hrs</strong></p>\n<p><strong> </strong></p>\n<p><strong>This is an equivalent of seventy-seven (77) hours per bi-weekly pay period!</strong></p>\n<p> </p>\n<p>We are looking for Community Support Workers (CSWs) to help us bring wrap-around patient-centered care to the <strong>Haliburton Region</strong>. <strong>Are you someone we should meet?</strong> </p>\n<h2>What We Offer</h2>\n<ul>\n <li><strong>Guaranteed 77 hours bi-weekly pay</strong></li>\n <li>Competitive wage grid <strong>$20.96 to $23.20 per hour</strong></li>\n <li><strong>PAID MILEAGE at 0.45 per/km</strong></li>\n <li>A special <strong>$500 to $750 bonus</strong> for signing with CarePartners</li>\n <li>Paid orientation and hands-on mentoring from experienced, caring staff</li>\n <li>Access to group discounts, including exclusive discounts from various retailers </li>\n <li>Total rewards program which includes health & dental benefits</li>\n <li>A robust Employee Assistance Program (EAP) for you and your family members</li>\n <li>Opportunities for continuing education and training</li>\n <li>Hands on mentoring and leadership support</li>\n <li>Opportunities to volunteer in countries with limited access to healthcare services</li>\n</ul>\n<h2>What The Role Involves</h2>\n<ul>\n <li>Contributing as a valued member of the interdisciplinary care team</li>\n <li>Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions</li>\n <li>Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety</li>\n <li>Providing cognitive, social and/or emotional support to the patient</li>\n <li>Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices</li>\n <li>Providing patient status reports and participate in patient/family care planning meetings</li>\n</ul>\n<h2>What You Bring</h2>\n<ul>\n <li>A strong desire and commitment to making a difference in the lives of their patients</li>\n <li>Exceptional interpersonal skills</li>\n <li>Ability to work both independently and as part of a team</li>\n <li>Experience working with patients with significant and complex care needs such as dementia and mental health</li>\n <li><strong>A valid driver’s license and vehicle for which you are insured</strong></li>\n <li>Community Support Worker (e.g.) Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or completion of first year Nursing program  with preference given to those with a PSW certificate and/or equivalency</li>\n <li>To protect our patients, we require current vulnerable sector check screening, and a clear background check</li>\n</ul>\n<h2>CarePartners In Your Community</h2>\n<p>In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
