Responsibilities
Job Summary
Support physicians, nurse practitioners, physician assistants, and/or nurses by taking vitals, assisting in medical examinations and other specific procedures and providing translation services.
Essential Job Functions
Assists participants and supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Recreational Coordinator.
Monitor and assist participants with meals at feeding table.
Assist throughout the day in maintaining clean and orderly day rooms, kitchen and personal care areas.
Accompanies enrollee to medical and other appointments.
Perform and assist with laundry, including clothes for participants.
Assists the drivers with the loading and unloading of participants from vehicles.
Demonstrates dependability through consistent compliance with scheduled work hours.
Participates in the activities of the interdisciplinary team as needed and carries out the plan of care as directed.
Note changes in participant's physical or emotional health and communicate this information to the appropriate staff person.
Maintains participants' medical record and fulfills VCC's PACE charting and reporting requirements.
Assists the transportation department with participants' take-home supplies.
Participates in and support Quality Improvement initiatives.
Follows all VCC's PACE Policies and Procedures and OSHA safety guidelines.
Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
Practices standard precautions and follows appropriate Infection Control procedures.
Maintains safe working environment. Follows VCC's PACE Safety Policies and Procedures.
Perform other duties as directed
Qualifications
Minimum Qualifications
High school graduate or equivalent
CPR Certification (including BLS Child, Infant, Adult)
Preferred Qualifications
Minimum of one years of experience in working with the frail or elderly population preferred.
Bilingusl English / Armenian
Required Skills/Knowledge/Abilities
Skilled in establishing and maintaining effective working relationships with participants, co-workers, and the public.
Basic knowledge in working with frail and elderly to support Activities of Daily Living.
Skilled in identifying and recommending problem resolution.
Ability to work independently with minimum supervision.
Ability to communicate clearly and effectively.
Ability to work effectively with cultural, economically, and educationally, diverse populations.
Preferred Skills/Knowledge/Abilities
Experience/familiarity with Practice Management and Electronic Medical Records systems, NextGen preferred
Pay Range
$23.00 - $26.00 Depending on Experience
