We even offer pet insurance for your four-legged family members.  </li>\n</ul>\n<ul>\n <li>Early access to earned wages with Daily Pay, tuition reimbursement to help further your career and adoption assistance to help you grow your family.   </li>\n</ul>\n<p>With a collective goal to champion a culture of belonging where everyone feels valued and respected, we honor the ways people are unique and embrace what brings us together.   </p>\n<p>And, we believe equipping you with support and development opportunities is a vital part of delivering an exceptional employment experience. </p>\n<p>Find a fulfilling career and make a difference with UnityPoint Health.</p>\n<h2>Responsibilities</h2>\n<p><strong>Clinical Excellence</strong> </p>\n<ul>\n <li>Facilitates the development of the interdisciplinary team plan of care to meet individual spiritual  needs. </li>\n <li>Maintain record of spiritual care services and related activities for quality assurance, program development, and policies and procedures. </li>\n <li>Document direct services and ongoing communication with community chaplains, spiritual counselors and hospice interdisciplinary team. </li>\n <li>Attend Interdisciplinary Team meetings and complete all associated documentation in an accurate and timely manner. </li>\n <li>Provide consultation, education, and support to the interdisciplinary group on spiritual care. </li>\n <li>Meets measures as determined and required for job productivity and performance improvement.  </li>\n <li>Provide bereavement follow up services as requested. </li>\n <li>Arrange for on-call availability of chaplaincy services. </li>\n <li>Conduct or make arrangements for funeral or memorial services for patients as requested. </li>\n <li>Assist in planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. </li>\n</ul>\n<p> </p>\n<p><strong>Collaboration </strong> </p>\n<ul>\n <li>Develop and maintain a resource group of clergy to whom specific aspects of spiritual care may be delegated. </li>\n <li>Recruit community chaplains, spiritual counselors, and spiritual care volunteers adequately to meet patient/ family/caregiver needs. </li>\n <li>Develop community contacts and offer education through congregations about hospice care. </li>\n <li>Provide educational programs for community clergy, religious and lay representatives as resources allow.  </li>\n <li>Provide educational in-services to the hospice and community. </li>\n <li>Serves as liaison and support to community chaplains and spiritual counselors.</li>\n</ul>\n<p><strong>Performance Improvement </strong> </p>\n<ul>\n <li>Completion of communication tasks and activities in a timely manner. This would include, but is not limited to: responding to email, voicemail or telephone messages, promptly, accurately, and professionally; attending staff meetings as scheduled or viewing videotapes of those meetings; asking questions of team members and supervisors when needing clarification about various day-to-day issues or patient needs; and reviewing employee communication pieces, to stay aware of UnityPoint at Home programs and initiatives. </li>\n <li>Participates in quality assurance and performance improvement activities as assigned. </li>\n <li>Perform other duties as requested by Hospice Administrator or Supervisor to facilitate the smooth and effective operations of the office. </li>\n <li>Maintain regular and consistent attendance at work. </li>\n <li>Behave in a manner consistent with Mission, Vision, Values and Expectations for Excellence. </li>\n <li>Maintain compliance with OSHA, Accreditation Standards and Risk Management guidelines</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education:</strong></p>\n<ul>\n <li>Graduate of accredited school of theology, or related Bachelor’s degree. </li>\n <li>2 units of CPE - can be completed within 2 years of hire</li>\n</ul>\n<p> </p>\n<p><strong>Experience:</strong> </p>\n<ul>\n <li>Ordination in a particular religious group and/or endorsed and in good standing from an ecclesiastical governing body </li>\n</ul>\n<p> </p>\n<p><strong>License(s)/Certification(s):</strong></p>\n<ul>\n <li>Valid driver’s license when driving any vehicle for work-related reasons. </li>\n <li>Valid Mandatory Reporter course completion by state(s) requirement  </li>\n <li>CPR Certification.</li>\n</ul>\n<p>#UPatHomeNursing </p>",
